What is the card transaction dispute form template?

Card disputes stall when the first report is missing something: which charge, why it's wrong, and whether the cardholder has already tried the merchant. This card transaction dispute form template asks the cardholder for the charge exactly as it appears on the statement, and then asks only the questions that fit their reason.

It's designed for credit unions, community banks, card programs, and businesses that issue corporate or prepaid cards. The cardholder enters only the last four digits of the card. The form never asks for the full number. Choosing I didn't make or allow this transaction opens a short fraud section: where the card is now, when they last used it, whether anyone else has used it with permission, whether they've shopped with the merchant before, and whether the card has been locked. Every other reason asks whether the merchant has been contacted, plus a detail that fits, such as the expected delivery date or the date a subscription was cancelled.

Each dispute is saved as a signed PDF in your FileIt vault, with receipts, emails and screenshots attached, ready for your disputes team to work.

The card transaction dispute form, page by page

1 Cardholder and transaction

The cardholder gives their name, email and phone. Card ending in accepts exactly four digits, with help text saying never to enter the full number. Card type is credit, debit or prepaid.

Under The transaction, the cardholder enters the merchant name as it shows on the statement, the transaction date, the date it posted, the transaction amount and the amount they're disputing, which may be part of the charge. Are you disputing other charges from the same merchant? opens a required box for the other charges, one date and amount per line.

2 Reason and evidence

Why are you disputing this charge? offers seven reasons, from I didn't make or allow this transaction to I was charged after cancelling a subscription.

An unauthorized charge opens a fraud section. Where is your card now? (still have it, lost, stolen, never received), when the cardholder last used it, whether anyone else has ever used the card with permission, whether they've shopped with the merchant before, whether they've locked the card or asked for a replacement, and a police report number if there is one.

Other reasons ask a fitting follow-up: when goods should have arrived, the date of a return or cancellation with a tracking number, what the amount should have been, or the date a subscription was cancelled. All of them ask Have you contacted the merchant to sort it out?, with the date and the merchant's reply on a yes.

Every dispute ends with Tell us what happened in your own words, an upload for receipts, emails, screenshots or proof of return, a declaration, signature and date.

Make the template yours

  • Rename the reasons to match the dispute categories your team works with
  • Add a field for the cardholder's member or account number
  • Add your dispute time limits to the cover text so cardholders know how soon to report
  • Change the currency on the amount fields
  • File disputes into separate folders for fraud and for merchant disputes
  • Add your disputes inbox to the notification emails

Tips for a better card transaction dispute form

  • Ask cardholders to copy the merchant name exactly as it appears on the statement
  • Encourage cardholders to contact the merchant first for non-fraud disputes, and to keep the reply
  • For unauthorized charges, lock or replace the card before anything else
  • Only ever ask for the last four digits of a card number on a form
  • Keep dispute folders restricted to the people who handle them

Every response becomes a PDF in your vault

Each dispute is saved as a signed PDF in the vault folder you choose, titled with the cardholder's name and the date, with every piece of evidence attached. Your team gets an email for each new dispute, and the cardholder can be sent a copy.

The Responses table lists every dispute with its reason and amount. Filter for unauthorized charges to deal with them first, and export to CSV for a log of disputes by merchant or by month.

  1. Start from this template. It opens in the FileIt Forms designer — change any question, add pages, set the rules for when questions appear.
