What is the bank account closure request form template?

Closing a bank account is simple until a direct deposit lands on the closed account or a forgotten subscription bounces. This bank account closure request form template asks the questions that prevent that, on one page, and finishes with a signature from every account holder.

It's for banks and credit unions taking closure requests online, and for people who want a written, signed request to send to the bank they're leaving. The customer gives the account details and a close date, and says how the remaining balance should be paid. Choosing Transfer it to another account opens the receiving bank, the name on the account, the routing number, the account number and the account type. A mailed check or collection at a branch needs nothing extra.

A few loose-end questions follow: items still waiting to clear, which payments have already moved, and whether the debit card and unused checks have been destroyed. For a joint account, the other holder's name and signature are required. Every request is saved as a signed PDF in your FileIt vault.

The bank account closure request form, page by page

1 Account closure request

The customer gives the account holder name, email, phone and mailing address, with help text explaining that the closing statement, and a check if they choose one, go there. They enter the account number and account type, from checking and savings to a CD or business account. Is this a joint account? is required, and a yes asks for the other account holder's name.

Close the account on or after sets the date. Main reason for closing offers moving away, fees, a better rate elsewhere, combining accounts, service or app experience, no longer needing it, or something else, with an optional box for what could have been done better.

Under Remaining balance, How should we pay out the remaining balance? offers a transfer to another account, a check mailed to the address above, or collection at a branch. A transfer asks for the receiving bank, the name on the receiving account, the routing number, the receiving account number, and whether it's checking or savings.

Loose ends asks whether any checks, card payments or deposits are still waiting to clear, and which ones if so. I've already moved lists direct deposits, automatic bill payments, debit card subscriptions and scheduled transfers. Have you destroyed the debit card and any unused checks? is the last question before the request to close, which every holder agrees to. The account holder signs, and on a joint account the other holder signs too.

Make the template yours

  • Change the routing number question to sort code, IBAN or SWIFT for accounts outside the US
  • Add the closing timeline and any fees to the cover text
  • Remove the feedback box if you collect exit feedback another way
  • Add a question about other accounts or loans the customer holds with you
  • File requests by month so operations can work through them in order
  • Add a member number field to make matching faster

Tips for a better bank account closure request form

  • Move direct deposits and automatic payments before asking to close the account
  • Leave the account open until every pending item has cleared
  • Download statements you'll need later, since access may end when the account closes
  • Check the receiving account details twice, especially the routing number
  • Keep the bank's closure confirmation with your other financial records

Every response becomes a PDF in your vault

Each request is saved as a signed PDF in your vault, titled with the account holder's name and the date, with both signatures on a joint account. You get an email for every request, and the customer can be sent a copy.

The Responses table lists every closure with the reason and payout method, so the CSV export doubles as a simple record of why customers leave.

  1. Start from this template. It opens in the FileIt Forms designer — change any question, add pages, set the rules for when questions appear.
  2. Share it. Turn on a public link, or send it to people by email, each with their own link. They don’t need a FileIt account.
  3. Get the answers as PDFs. Each response is saved as a PDF in the vault folder you choose, with uploaded files attached — and listed in a Responses table you can export to CSV.
