What is the brand discovery questionnaire form template?

A logo designed without understanding the business behind it is a guess. Before any design work, a branding studio needs to know why the business exists, who it serves, who it competes with, how it wants to be perceived, and what the client already has — or wants to leave behind.

This brand discovery questionnaire gathers that in five short sections. Clients describe the business and whether this is a new brand, a refresh or a rebrand; their purpose, values and ideal customer; their competitors and what sets them apart; their brand personality through three words and five simple sliders; and their visual preferences, inspiration and the deliverables they need.

1 About the business

The client gives their contact details, business name, website, industry and year founded, then explains what the business does in two or three sentences. They say whether this is a brand-new brand, a refresh, a complete rebrand or a new sub-brand; anything other than a brand-new brand asks why now, and what isn't working.

2 Purpose & audience

The client explains why the business exists beyond making money, lists three to five values, and describes their ideal customer, the problem they solve and how customers describe them today.

3 Market & positioning

The client names their main competitors, says what makes them different, lists brands they admire in any industry, and places themselves on price — budget, mid-market, premium or luxury.

4 Brand personality

Three words they'd like people to use about the brand come first. Five 1-5 sliders then place the brand between classic and modern, serious and playful, understated and bold, approachable and exclusive, and simple and detailed. Two questions ask how people should feel on meeting the brand, and what it should never feel like.

5 Look & feel

For refreshes and rebrands, the client lists what must be kept and uploads their current logo or materials. Everyone gives colours they like and want to avoid, ticks visual styles that appeal (minimal, vintage, hand-drawn, geometric, corporate, luxurious, playful or natural), and can upload up to ten inspiration images. The form finishes with the deliverables they need, a deadline and a budget band.

Tips for a better brand discovery questionnaire form

  • Send the questionnaire a few days before the discovery call so clients have time to think
  • Look for differences between what clients say about their audience and their competitors
  • Ask about anything that surprised you in the answers during the workshop
  • Treat inspiration images as a conversation starter, not a specification
  • Play the three words back in your first presentation — it shows you listened
  • Keep the completed questionnaire with the project files for future brand work

Every response becomes a PDF in your vault

Each completed questionnaire is saved as a PDF in your vault with the current logo and inspiration images attached. With notifications on, you're emailed when it arrives, and the client receives a copy of their answers.

The Responses table keeps every client's answers together, and you can export them to a CSV. Many studios print the PDF or share it on screen during the discovery workshop.

Do clients need an account?

No. Clients use the link to fill in and submit the questionnaire.

What files can clients upload?
