What is the bowling league sign-up form template?
Bowling leagues are usually built from two kinds of sign-up: a captain who turns up with a full team, and a bowler on their own who just wants a spot. This template handles both from one link. The first question — 'How are you signing up?' — decides the rest of the page.
Captains name the team and fill in a roster: bowlers two to five with their averages, an optional substitute and everyone's shirt size. Individuals see none of that. Instead they're asked what kind of team they'd like, casual or competitive, and whether there's anyone they'd like to bowl with. Everyone gives their own average and shirt size, so the league secretary can work out handicaps and order shirts.
The second page covers the league itself: preferred night and any other nights that could work, the league type from handicap to no-tap, an association membership number if your league is sanctioned, whether anyone is new to league bowling, and how fees will be paid. Your league details and rules go in a placeholder paragraph that bowlers agree to. Each sign-up is saved as a PDF in your FileIt vault.

The bowling league sign-up form, page by page
1 Bowler & team
Your name, email and mobile, and a required 'How are you signing up?'. Everyone can give their current average and shirt size. Captains add a required team name and a roster: bowlers 2 and 3 are required, bowlers 4 and 5 optional, each with an average, plus a substitute and teammates' shirt sizes. Individuals instead say what kind of team they'd like and who they'd like to bowl with — the roster stays hidden.
2 League details
A required preferred league night and other nights that could work, a required league type, an association membership number, whether anyone is new to league bowling, and a required fee payment choice. A notes box for weeks away, your league details placeholder and a required agreement to the league rules.
Make the template yours
- Replace the placeholder with your lineage, prize fund, season dates and rules
- Change the nights and league types to the leagues your centre runs
- Add bowlers 6 and 7 if your leagues use larger teams
- Remove the association number if your leagues aren't sanctioned
- Set a close date so sign-ups stop before the organisational meeting
- Cap responses at the number of team slots you have for each night
Tips for a better bowling league sign-up form
- Export the Responses table to CSV and sort by night to see which leagues are filling
- Group individual bowlers by their team preference before placing them
- Ask captains to confirm averages from last season's standings sheet
- Order shirts only after the first night, when rosters have settled
- Send the link again a week before the season for late sign-ups
Every response becomes a PDF in your vault
Each sign-up is saved as a PDF in your FileIt vault, and the league secretary can be emailed for every one. Captains and bowlers see your confirmation and can receive a copy.
Use the Responses table to filter captains and individuals, then search for a team name when someone asks whether their team is in.
- Start from this template. It opens in the FileIt Forms designer — change any question, add pages, set the rules for when questions appear.
- Share it. Turn on a public link, or send it to people by email, each with their own link. They don’t need a FileIt account.
- Get the answers as PDFs. Each response is saved as a PDF in the vault folder you choose, with uploaded files attached — and listed in a Responses table you can export to CSV.
