What is the fantasy football league sign-up form template?
Every commissioner knows the pre-season scramble: who's in, who's out, what's everyone's username, and which Saturday can twelve people actually agree on for the draft. This one-page sign-up template collects it from each manager in the same format, so you can send invites, set the draft and settle the keeper question with the answers in front of you.
Managers give their name, email and an optional mobile for the group chat, a team name, whether they played last season, the platform they'll use and their username on it. The draft day section asks when they could draft, dates they definitely can't make, and whether they prefer an in-person or online draft, snake or auction, and standard, half-PPR or full PPR scoring. The keeper preference runs from redraft to dynasty, and returning managers can list the players they're thinking of keeping.
Then the league type. Free leagues are bragging rights only. Choosing 'Paid league' reveals the buy-in amount, how the manager will pay and the date they'll pay by. Your league rules go in a placeholder, and every manager agrees to the commissioner's decisions and to setting a lineup each week. Each sign-up is saved as a PDF in your FileIt vault.

The fantasy football league sign-up form, page by page
1 League sign-up
Your name and email (required), an optional mobile, a required team name, 'Did you play in this league last season?', platform and username. Draft day: required days you could draft, dates you can't, and preferred draft format, snake or auction, and scoring. A required keeper preference — returning managers can list possible keepers. A required 'Free or paid league?'; paid leagues ask for the buy-in amount and payment method (required) and a pay-by date. Your league rules placeholder and two required agreements: the rules and commissioner's decisions, and setting a lineup every week.
Make the template yours
- Replace the placeholder with your roster, trade, waiver, playoff and payout rules
- Remove the free-or-paid question if your league is always free
- Add your own draft date options instead of days of the week
- Change the platform list to the one your league uses and drop the dropdown
- Turn the keeper question into a vote for a rules change you're considering
- Set the form to close a few days before the draft
Tips for a better fantasy football league sign-up form
- Export the Responses table to tally draft-day availability before announcing a date
- Count keeper votes with the Responses filters rather than in the group chat
- Send platform invites as soon as each username arrives
- Keep each season's sign-ups in their own vault folder for the league history
- Check what's allowed where you and your managers live before running a paid league
Every response becomes a PDF in your vault
Each sign-up is saved as a PDF in your FileIt vault and the commissioner can be emailed for every one. Managers see your confirmation and can receive a copy.
The Responses table lists every manager's team name, username and preferences; export it to CSV to plan the draft order or track who has paid.
- Start from this template. It opens in the FileIt Forms designer — change any question, add pages, set the rules for when questions appear.
- Share it. Turn on a public link, or send it to people by email, each with their own link. They don’t need a FileIt account.
- Get the answers as PDFs. Each response is saved as a PDF in the vault folder you choose, with uploaded files attached — and listed in a Responses table you can export to CSV.
