What is the athlete recruiting profile form template?
College coaches see hundreds of emails from hopeful athletes, and the ones that get a reply usually have the same things in common: the right stats for the sport, a highlight video, grades and test scores, and a coach who will vouch for the player. This template collects all of that in a set order, so a high-school coach, club director or recruiting adviser can build a consistent profile for every athlete they help.
The first page covers the athlete: name, date of birth, graduation year, contact details, school, hometown, height, weight, a parent or guardian's contact and an optional profile photo. The second page asks for the primary sport, position or events, teams, years on varsity, jersey number, key stats from last season, measurables and personal bests, a highlight video link and honors. Answering yes to 'Do you play a second sport?' repeats the whole stats section for that sport.
The third page is academics and references — GPA and scale, SAT or ACT scores, intended major, an eligibility centre ID, advanced courses, an optional unofficial transcript upload and the college levels they're interested in — plus one or two coach references and a short note to college coaches. Each completed profile is saved as a PDF in your FileIt vault, ready to attach to an email.

Tips for a better athlete recruiting profile form
- Ask athletes to update the profile after each season rather than starting over
- Check stats against official team records before a profile goes out
- Tell references in advance that college coaches may contact them
- Keep highlight videos short, with the athlete's best plays first
- Re-send the form to the athlete when grades or test scores change
Every response becomes a PDF in your vault
Each profile is saved as a PDF in your FileIt vault, with the photo and transcript stored as attachments. The athlete can receive a copy by email.
Use the Responses table to filter by graduation year or sport, and open any profile's PDF to attach it to an email to a college coach.
- Start from this template. It opens in the FileIt Forms designer — change any question, add pages, set the rules for when questions appear.
- Share it. Turn on a public link, or send it to people by email, each with their own link. They don’t need a FileIt account.
- Get the answers as PDFs. Each response is saved as a PDF in the vault folder you choose, with uploaded files attached — and listed in a Responses table you can export to CSV.
