What is RP 5068 old-age pension claim?

RP 5068 is the claim form for the Pensão de Velhice, the old-age pension paid by Portugal's Social Security (Segurança Social, run on the mainland by the Instituto da Segurança Social). It covers both the pension from the normal pension age and the early pension (Pensão Antecipada de Velhice) available under several schemes: flexible retirement age, a very long contribution record, arduous or strenuous occupations provided for by law, economic reasons or pre-retirement, long-term unemployment, and disability.

Social Security asks people to claim preferably online through Segurança Social Direta, which it says is faster. The paper form is still accepted: it can be filled in on a computer and handed in at, or posted to, a Social Security service. Either way the claim can go in at most 3 months before the date you want the pension to start. Besides your identity and address, the form asks about your contribution record in the civil service scheme, abroad, in military service and in the lawyers' and solicitors' fund, other pensions you receive, and your family and disability situation for income tax withholding.

Who needs to fill out RP 5068 old-age pension claim?

• People insured with Portuguese Social Security who are reaching the normal age of access to the old-age pension and want their pension to start.
• People who qualify for an early old-age pension — under flexible retirement age, a very long contribution record, arduous occupations, pre-retirement, long-term unemployment or disability.
• People who also worked in other countries, including EU countries and countries with a bilateral agreement, and want their periods there taken into account or a pension claimed there.
• People who were in the civil service scheme (Caixa Geral de Aposentações) and may want a unified pension.
• Former lawyers and solicitors covered by the Caixa de Previdência dos Advogados e Solicitadores.
• Relatives helping an older family member prepare a paper claim — the claimant still signs, or someone signs at their request if they cannot.

When to use RP 5068 old-age pension claim

• Up to 3 months before the date you want your old-age pension to start — not earlier.
• When you decide to take an early pension: the form asks which scheme applies and, for flexible retirement age, whether you are still working and when you will stop (within 60 days of the claim).
• It is a one-off claim. Once the pension is awarded, you undertake to report changes to the information you gave rather than filling in a new RP 5068.

What you need before you start

• Your Social Security identification number (NISS), with 11 digits.
• Your Cartão de Cidadão, Bilhete de Identidade or passport; without a Cartão de Cidadão, also your address and tax identification document.
• The date you want the pension to start and, for an early pension, the scheme you are claiming under.
• For a flexible-age early pension while working: your occupation and the date you will stop, plus form RP 5023 (declaration of the professional activity) for an early pension.
• The countries you have worked in and your social security numbers there; for a claim abroad, form RP 5071, and for EU or agreement cases also RP 5081.
• Proof of compulsory military service if you haven't asked the Caixa Geral de Aposentações to count it.
• The monthly amount and payer of any other invalidity or old-age pension you receive.
• A multi-purpose medical disability certificate (Atestado Médico de Incapacidade Multiusos) for you or your spouse, if either of you has a disability of 60% or more; 80% or more for the early pension on disability grounds.
• A residence document if you are a foreign national, and proof of any special scheme you mention.

How to fill out RP 5068 old-age pension claim, step by step

1 1.1 Identificação

Your full name, NISS and date of birth, a phone number and email. The address lines (morada, locality, postcode, parish, municipality and district) are filled in only if you don't have a Cartão de Cidadão; if you do, Social Security uses the address on your card, and quadro 6 explains how to register a different one through the ePortugal portal, a Espaço Cidadão or a Cartão de Cidadão service desk.

2 1.2 Modalidade de Pensão

Choose the old-age pension with its start date — adding any special situation or scheme that applies to you — or the early old-age pension with its start date and the scheme you claim under ('ao abrigo de').

For an early pension for flexible retirement age with a provisional amount, say whether you are working, your occupation and the date you will stop (you must stop within 60 days of the claim; a member of a company's statutory body attaches their resignation from management). Then accept a provisional amount based on your current record — checked later and possibly changed in the final calculation — or ask for the final calculation instead. The part ends by asking whether you want to claim the pension only abroad.

3 2 Carreira Contributiva

2.1 asks whether you are or were covered by the Caixa Geral de Aposentações and, if so, whether you want a unified pension. 2.2 asks whether you have worked in another country: list up to three countries (A, B, C) with your social security number in each, then say whether you already receive a pension from any of them and whether you want to claim one there — in which case you also fill in form RP 5071.

2.3 covers compulsory military service in the Portuguese Armed Forces and whether you asked the Caixa Geral de Aposentações to count it. 2.4 asks about the lawyers' and solicitors' fund (CPAS) and your beneficiary number there.

4 3 Rendimentos atuais de Pensões

If you already receive another invalidity or old-age pension, give its monthly amount in euros and the body that pays it, on the row for its origin: occupational disease, accident at work, foreign social security, civil service or other amounts. Then say whether you have applied for another such pension, and to which body.

5 4 Dados para efeitos de IRS

Used for the income tax withheld from your pension. 4.1: married with only one of you having a pension or income, married with both having one, or not married. 4.2: whether you have a disability (and, if so, whether it comes from the Armed Forces) and whether your spouse does. A disability of 60% or more is proved with the multi-purpose medical certificate.

6 5 Declarações

You undertake to report any change to the information you gave and declare that it is complete and true, then date and sign. The signature must match your valid identity document. If you cannot or do not know how to sign, another person signs at your request (a rogo) and their identity document is attached.

7 6 Informações

Pages 3 to 5 explain the kinds of old-age pension, the documents to attach, how pensions are paid to the IBAN registered with Social Security (registered or updated in Segurança Social Direta or with form MG 14), how to change your address, and where and when to deliver the claim. The box at the end of page 5 is for Social Security staff only.

Common mistakes to avoid

• Claiming more than 3 months before the pension's start date — Social Security accepts the claim only within that window.
• Filling in the address although you hold a Cartão de Cidadão. The card's address is used; change it through the channels in quadro 6 if needed.
• Choosing the early pension without naming the scheme in 'ao abrigo de', or without the supporting documents such as RP 5023.
• For a flexible-age early pension, giving a stop date more than 60 days after the claim.
• Ticking a pension or claim in country B or C without naming that country above.
• Forgetting form RP 5071 when you want to claim a pension in another country you worked in.
• Not checking the IBAN registered with Social Security — pensions are paid to it, and the form has no bank box.

After you fill it out

Sign the claim as on your identity document — by hand, or through FileIt's e-signature if you prefer, then print it. Hand it in or post it to a Social Security service together with the documents listed in quadro 6. Alternatively, Social Security's preferred route is to submit the claim and documents in Segurança Social Direta. FileIt does not send the form anywhere.
