What is GDP 17 death benefits (occupational disease)?

GDP 17 (Requerimento de Prestações por Morte) belongs to Portugal's protection for occupational diseases (Proteção na Doença Profissional). When someone dies of an occupational disease, their family can claim a survivor's pension (Pensão por Morte) and a death grant (Subsídio por Morte), and whoever paid for the funeral or the transfer of the body can claim the funeral expenses allowance (Subsídio por Despesas de Funeral).

The four-page form identifies the deceased and the applicant, records your relationship to the deceased (spouse or partner, divorced or separated spouse entitled to maintenance, child, grandchild or ascendant), lists up to nine other family members who are also entitled, and has a separate part for funeral costs. Page 3 lists the documents to attach and the deadlines.

FileIt explains each question in English, lets you tick more than one benefit, shows tables 3 and 4 only when you claim the pension or grant, checks the deadlines the form gives, and prints your answers onto Social Security's own GDP 17/2025, keeping accented names intact. The PDF is saved in your vault.

How to fill out GDP 17 death benefits (occupational disease), step by step

1 What you are applying for

Tick the survivor's pension, the death grant, the funeral expenses allowance, or any combination. The form says a funeral-only applicant leaves out tables 3 and 4.

2 1. The deceased

Full name, NISS, date of birth and date of death. FileIt prints dates in the form's year-month-day cells.

3 2. Your details

Your name, NISS, date of birth, full address with postcode, municipality and district, phone and email.

4 3. More about you

Your relationship to the deceased. A divorced or separated spouse says whether they were entitled to maintenance, and a grandchild or ascendant says whether they were a dependant. Children and grandchildren aged 18 or over give their level of education. Then say whether you get, or have applied for, death benefits from another mandatory scheme, such as the Caixa Geral de Aposentações or a foreign body.

5 4. Other people entitled

Up to nine other family members, each with name, NISS, date of birth, relationship, and the education or dependant columns where they apply.

6 5. Funeral expenses

Say whether it was a funeral or a transfer of the body, give the date and the amount you paid in euros.

7 6. Statements and signature

You acknowledge that Social Security may check your tax information, that false statements are punishable, and that you will repay amounts received after the death. Date the form and sign it, or have someone sign on your behalf if you can't.

Common mistakes to avoid

  • Filling in tables 3 and 4 when you claim only the funeral allowance.
  • Claiming the pension or grant more than 5 years after the death, or the funeral allowance more than a year after the expense.
  • Forgetting annex GDP 17/1 when the deceased never applied for incapacity.
  • Sending a copy of the funeral receipt when the form asks for the original and an itemised invoice.
  • Using GDP 17 for a death that wasn't from an occupational disease; the general-scheme survivor's benefits use other Social Security forms.

After you fill it out

Sign the form and send it with the required documents through Segurança Social Direta or by post to a Social Security service, as the form says. FileIt doesn't send anything for you.

Payments go to the IBAN Social Security has on file. If you haven't registered one, do it on Segurança Social Direta or with form MG 14.

Keep the filled PDF in your FileIt vault together with the death certificate and the funeral invoices.
