What is RV 1006 - NISS for a foreign citizen?

The NISS (Número de Identificação de Segurança Social) is your number in Portugal's social security system. You need it to work and pay contributions, and to claim benefits. Foreign citizens apply for one with the Segurança Social. Form RV 1006, in its 2025 edition, is the paper application. The Segurança Social also takes NISS requests through its own online services.

The form asks for your names exactly as in your civil register, including your birth surname and all your given names in order, and your parents' names. It also asks for your sex, marital status, date of birth and nationality, plus the identity document you will show and its expiry date. Then come your place of birth, your social security number in your country of nationality if you have one, and your address in Portugal. Its notes explain special cases, such as Paris arrondissements and the compulsory province or department of birth for French and Italian nationals.

How to fill out RV 1006 - NISS for a foreign citizen, step by step

1 1.1 Nome e apelido

Family surname, including a surname acquired by marriage, then birth surname ('IDEM' if the same), then all your given names in civil-register order.

2 1.2 Filiação

Your father's and mother's surnames and given names.

3 1.3 Outros elementos

Sex, marital status, date of birth (year, month, day), nationality, the identity document with its number and expiry date, and your phone and email.

4 Local de nascimento

Town (with the arrondissement for French cities split into them), province or department (compulsory for French and Italian nationals), country, your foreign social security number, and any remarks.

5 1.4 Morada

Your address, locality, postcode (0000-000) with postal locality, district, municipality and parish.

6 2 Certificação

Date the form and sign it by hand as on your identity document, declaring that the information is complete and true. Someone else can sign at your request if you can't.

Common mistakes to avoid

• Leaving out the birth surname. The form's notes say it must always be given ('IDEM' if the same as the family surname).
• Writing given names out of order or shortening them. The notes ask for all of them in civil-register order.
• Forgetting the province or department of birth, which is compulsory for French and Italian nationals.
• Using an expired identity document. The form asks for a valid one.
• Signing differently from your identity document.

After you fill it out

Print the PDF and sign it by hand. FileIt does not send it for you.
