What is Jamaica adult passport renewal (simplified)?

The Jamaica Adult Renewal Passport Application is the simplified form the Passport, Immigration and Citizenship Agency (PICA) offers to adults who are renewing an existing Jamaican passport. Instead of the full passport application, with its long sections on parents, citizenship and countersigning, it asks only for your personal data, contact details, emergency contacts and the particulars of the passport you are renewing, followed by your declaration, photographs and thumbprint.

The form is short, but it can only be used if every condition on PICA's instruction sheet is met. If even one of them doesn't apply to you — for example you have changed your name or your marital status since your passport was issued — you need the full Jamaican Passport Application Form instead. Getting this right before you hand anything in saves a wasted trip to a PICA office, embassy or consulate.

Who needs to fill out Jamaica adult passport renewal (simplified)?

• Jamaican citizens who were 18 or over when they applied for the passport they now want to renew.
• Holders of a ten-year Jamaican passport issued after September 2001.
• People whose name, date of birth and place of birth on the new application will be exactly as they appear in the current passport.
• Applicants whose marital status has not changed since the current passport was issued.
• Jamaicans living overseas who will apply through their nearest Jamaican embassy, consulate or high commission.
• Anyone whose passport is undamaged and has never been reported lost or stolen — otherwise the simplified form cannot be used.

• When your current passport is valid for no more than one more year.
• Earlier than that only if all the visa pages in your passport have been used.
• When your passport has expired, as long as it expired no more than two years ago.
• Before booking travel, so there is time for processing — check PICA's current turnaround times rather than relying on an estimate.
• Not after a name change, a change in marital status, or if your passport was lost, stolen or damaged: those cases need the full application form.

What you need before you start

• Your current Jamaican passport — the one being renewed. It must be submitted with the application.
• Two identical, professionally produced colour photographs no more than six months old, taken against an off-white background (pale blue for grey, white or bleached hair), with no embossed seal.
• Your passport number, place of issue and date of issue.
• Your full name, maiden name if any, date of birth, and town, city or parish and country of birth exactly as in your passport.
• Details of your last marriage (spouse's name, date, place and country) if you are married, widowed, divorced or separated.
• Your permanent and mailing addresses, telephone numbers and email address.
• Names, relationships, addresses and phone numbers of up to two emergency contacts who know you.
• The applicable fee — check PICA or your embassy or consulate for the current amount.
• If someone else will hand in your application, a notarised consent letter and their valid identification.

How to fill out Jamaica adult passport renewal (simplified), step by step

1 Section A — Personal data

Your last name, maiden name (your surname at birth, if different), given names, date of birth, place of birth (town, city and parish) and country of birth. These must match your current passport exactly — the simplified renewal is not for correcting or changing them.

You also give your profession or occupation (the form warns that more than 18 characters will be abbreviated in the passport), any special visible features such as a scar, and your marital status: single, divorced, married, widowed or separated.

2 Particulars of last marriage

If you are married, widowed, divorced or separated, give your spouse's full name and the date, place and country of your last marriage. Leave this blank if you are single.

Remember that the simplified form is only for people whose marital status is the same as when their current passport was issued.

3 Section B — Contact information

Your permanent address, a mailing address if it is different, and your home, business and cell phone numbers, plus an email address. The form prints Jamaican and other North American numbers as (area code) (xxx-xxxx); FileIt splits them into those blanks for you, and writes overseas numbers in full.

This section also asks for emergency contacts: people who know you, with their full name, relationship to you, address, telephone number and email. The form has room for two.

4 Section C — Particulars of current passport

The number of the passport you are renewing, where it was issued (city, state, country) and its date of issue. FileIt uses the issue date to check the instruction sheet's rules — a passport issued before October 2001, one that expired more than two years ago, or one issued before you turned 18 all point you to the full application form instead.

5 Section D — Religion/sect

Only needed if you must wear head gear in your photograph for religious reasons. The instruction sheet says no head covering is allowed in passport photographs except for religious purposes.

6 Section E — Declaration of applicant

You declare that the information is correct and that every previous passport granted to you has been surrendered, other than any passport or travel document you name here. Leave the number blank if you have nothing else to declare.

Attach the two photographs in the boxes, sign inside the signature box — the form warns that the application is void if your signature touches the border — put your thumbprint in the thumbprint box, and date the declaration.

7 Section F — Supplementary information

Six lines for anything else PICA should know, such as why you are renewing early. FileIt wraps your text onto the lines and tells you if it is too long to fit.

8 Section G — For official use only

Completed by PICA staff when your documents are received and your passport is produced. Leave it blank.

Common mistakes to avoid

• Using the simplified form when one of the conditions isn't met — a changed name or marital status, a lost or damaged passport, or a passport issued when you were a minor all need the full application.
• Writing your name or place of birth differently from your current passport, even with a small spelling or order change.
• Letting your signature touch the border of the signature box, which makes the application void.
• Forgetting the thumbprint, or smudging it across the box lines.
• Sending photographs that are older than six months, not identical, taken against the wrong background, or with an embossed seal.
• Leaving the marriage particulars blank when you are married, widowed, divorced or separated.
• Not enclosing your current passport and the fee, especially when applying by registered mail.

After you fill it out

Print both pages, sign inside the signature box without touching its border, add your thumbprint and the date, and attach two identical photographs. The signature and thumbprint have to be made on paper, so there is no electronic signature for this form.

According to PICA's instruction sheet, you can submit the application in person, through a third party (with a notarised consent letter and their valid ID) or by registered mail. In Jamaica, mail it to the Director of Passport Services at PICA's head office on Constant Spring Road, Kingston 10; overseas, send it to your nearest Jamaican embassy, consulate or mission. Include your current passport, the photographs and the fee. PICA may ask for more documents or ask you to attend in person.
