What is PPA identity document application (passport / ID card)?

The Application for Identity Documents (in Estonian, isikut tõendavate dokumentide taotlus) is the Police and Border Guard Board's two-page paper form for Estonian identity documents: the ID card or residence permit card, a travel document (Estonian citizen's passport, alien's passport, refugee's travel document or temporary travel document), the Estonian citizen's additional passport and the seafarer's discharge book. The English version is the one the PPA links as the "ID card application form" for people applying at an Estonian foreign mission.

Most adults renewing a document while living in Estonia use the PPA self-service portal instead, so this paper form matters mainly when you apply in person: at an Estonian embassy or consulate abroad, or at a PPA service office. It must be completed in capital letters, with names written in Latin letters exactly as in your identity document, and it must be signed on both pages. Corrections are not allowed, so a clean, typed copy saves a second visit.

Who needs to fill out PPA identity document application (passport / ID card)?

Estonian citizens living abroad who apply for a new ID card or passport at an Estonian embassy or consulate.
Anyone applying for an Estonian citizen's additional passport, a second passport for citizens who already hold one.
Foreign nationals with no travel document of their own who apply for an Estonian alien's passport.
Parents or guardians applying for a child under 15, and legal representatives of an adult with restricted active legal capacity.
Seafarers applying for a seafarer's discharge book or certificate of record of service on ships.
Citizens abroad who need a one-year Estonian passport because they are far from any Estonian representation.

When to use PPA identity document application (passport / ID card)

When your ID card or passport is about to expire and you are applying in person rather than through the self-service portal.
After your document is lost, destroyed or stolen, or becomes unusable.
When your personal data has changed, for example after a change of name.
For a first application, such as a child's first Estonian passport or ID card.
When you need a short-term, one-year passport because reaching an Estonian foreign representation would be disproportionately burdensome.

What you need before you start

Your current identity document, which you take with you when you apply.
Your Estonian personal code (isikukood), or your date of birth if you don't have one.
Your country of birth and citizenship(s), and optionally your education, nationality and native language.
A contact address, zip code, email address and phone number.
The name of the embassy, consulate or service office where you will collect each document.
A digital colour photo of at least 1300 x 1600 pixels, JPG, 1 to 5 MB, as the form asks.
For a child or a represented adult: the legal representative's name and personal code or date of birth, and an institution's register code if an institution represents them.
Proof that the state fee has been paid, and any documents the PPA asks for (for example proof of citizenship on a first application or documents about a name change).

How to fill out PPA identity document application (passport / ID card), step by step

1 Personal data

Give your given names and surname exactly as in your identity document, in Latin letters. The next box takes either your Estonian personal code or, if you have none, your date of birth (dd/mm/yyyy): FileIt prints whichever applies. Country of birth should be the country's current name. Education, nationality and native language are optional, marked with an asterisk on the form.

2 Contacts

Your contact address goes on one line in the order the form gives: street or farm, house and apartment number; village, borough or city; parish; county; country. Add the zip code, email and phone number so the PPA can contact you about the application.

3 Documents being applied for and places of issue

Tick each document you want: identity card or residence permit card, travel document, Estonian citizen's additional passport, or seafarer's discharge book. For each, choose the reason (first application, expiry, data changed, lost / destroyed / stolen, document unusable) and the pick-up location.

Documents are issued to the applicant, the legal representative or an authorised person, so choose a place where one of you can collect it.

4 Estonian citizen's passport with a validity of one year

Only for a one-year passport. Tick one of the two confirmations: that you are staying in a country with no Estonian foreign representation, or that it is disproportionately burdensome for you to go to one. 5 Alien's passport and the National Defence Act

If you are applying for an alien's passport, confirm that you have no travel document of a foreign country and no possibility of getting one. A separate box is for people staying in Estonia with a permit issued under the National Defence Act.

6 Legal representative

An application for a child under 15, or for a person with restricted active legal capacity, is submitted by the legal representative. Give the representative's name and personal code or date of birth. If an institution acts as the representative, add its name and register code. Applicants aged 15 or over can apply on their own.

7 Confirmation, date and signatures

The applicant (or legal representative) confirms that the data is correct and that giving incorrect data is punishable, and, for an ID card or residence permit card, agrees to the terms of use of certificates. FileIt prints the date at the end of both pages.

Sign by hand at the end of page 1 and page 2. The specimen signature box on page 1 is signed by applicants aged 15 or over in dark ink without crossing the borders; children aged 7 to 14 may sign, and it is left blank for children under 7.

Common mistakes to avoid

Writing names differently from the identity document, for example leaving out a second given name or diacritics.
Entering both a personal code and a different date of birth: the box takes one or the other, and they must agree.
Forgetting to choose a reason or a pick-up location for each document ticked.
Signing only page 1: the form asks for a signature on both pages as well as the specimen signature.
Letting the specimen signature touch or cross the border of its box.
Making corrections on the printed form, which the PPA doesn't allow; print a fresh copy instead.
Forgetting to send the digital photo separately, or sending one below the minimum size.

After you fill it out

Print both pages, sign at the end of each page and write your specimen signature inside the box on page 1. Take the form in person to the Estonian embassy or consulate, or the PPA service office, with your identity document and proof of the state fee. Fingerprints are taken there if the PPA needs new ones.

What is the PPA Application for Identity Documents?

Can I apply for an Estonian passport abroad with this form?

Do I need this form if I renew my ID card online?

No. An adult renewing in the PPA self-service portal doesn't use the paper form. It is for applications made in person.

What if I don't have an Estonian personal code?
