What is the logo & trademark use request form template?
Once a brand is known, people want to use it: customers adding you to a 'trusted by' wall, event organizers printing sponsor banners, journalists illustrating an article, partners planning co-branded campaigns. Requests arrive by email with half the details missing, and nobody is sure who approved what.
This template captures the request properly. It asks who's asking and their relationship with you, which assets they want — logo, name, product icons, screenshots or press photos — the purpose, a description of how and where it will appear, the channels, regions and dates, and whether anything with your logo will be sold.
Requesters can upload a mock-up and agree to follow your brand guidelines if approved. A note makes clear that nothing is permission until you confirm in writing. Each request is saved as a PDF in your FileIt vault, so you have a record of every approval.

1 About you
Name, organization, email and 'Your relationship with us' are required; job title, phone and website are optional. Relationships include customer, partner or reseller, sponsor or event organizer, supplier, media or publisher, school or nonprofit, or another.
2 Proposed use
'What would you like to use?' — our logo, our name in text, product names or icons, product screenshots, photos from our press kit, or something else. 'Purpose' ranges from a customer or partner logo wall and co-branded marketing to events, articles and teaching material, packaging or merchandise, and advertising. 'Describe how and where it will appear' is required. 'Where will it appear?' covers website, social media, print, video, products or packaging, event signage and presentations. Start date is required; end date and regions are optional. 'Will anything with our logo on it be sold?' is required, and yes asks what, how many and at what price. A mock-up or draft can be uploaded.
3 Agreement
A note explains that each request is reviewed individually and nothing is permission until confirmed in writing. The requester agrees to follow your brand guidelines and stop using the assets if asked, dates the request and can sign.

Every response becomes a PDF in your vault
Each request is saved as a PDF titled with the requester's name in the vault folder you choose, with any mock-up attached. The requester receives a copy by email.
Search the Responses tab by organization or purpose, and export to CSV for a running log of who has asked to use your brand.
