What is the copyright infringement notice form template?

If people can post content on your website, marketplace, forum or app, sooner or later someone will report that their work has been copied. Reports that arrive by email often leave out the essentials: which work, which page, and who is making the claim. A structured form gets the right information first time.

This template asks who is reporting and whether they're the owner or acting for one, with full contact details. It then asks about the original work — its type, a description, where to see it, any registration and optional evidence — and the material being reported: the exact links, how it uses the work, any contact with the poster, and what action is wanted.

The reporter ticks good-faith, accuracy and sharing statements and signs. Each notice is filed as a PDF in your FileIt vault, giving you a dated record of what was reported and when.

1 About you

Name, email, phone and postal address are required. 'You're reporting as…' — the copyright owner, or someone authorized to act for the owner; the latter requires the owner's name. Company or organization is optional.

2 The original work

'Type of work' — photo or image, written text, video, music or audio, software or code, artwork or design, or other — and 'Describe the original work' are required. The reporter can add a link to the original, a registration number and up to five files of evidence of ownership.

3 The material you're reporting

'Exact link(s) to the material' is required, one per line, with help text explaining that a whole profile or search page isn't enough. 'How does it use your work?' — copied in full, copied in part, changed or adapted, or used in a product for sale. 'Have you contacted the person who posted it?' is required; yes asks what happened. 'What would you like us to do?' — remove the material, add a credit or link, or something else.

4 Declarations

Three required statements: good faith that the use isn't authorized by the owner, its agent or the law; accuracy, and that the reporter is the owner or authorized; and that the notice, including contact details, may be shared with the person who posted the material. A signature and date complete it.

Tips for a better copyright infringement notice form

  • Act on the exact links given; don't remove more than was reported
  • Tell the poster what was reported and how they can respond
  • Keep a log of repeat reports against the same account
  • Acknowledge every notice, even if you need more information
  • Check notices for obvious misuse, such as reports about criticism or reviews
  • Keep notices for as long as your process requires
