What is the pilates studio intake form template?

A good Pilates teacher adjusts every session to the person in front of them, and that starts with a proper intake. This template asks new clients what a studio needs to know — their experience, what they want from Pilates, any injuries or conditions, and whether they're pregnant or recently postnatal — and finishes with your studio policy and a signature.

Page one covers contact details and 'Your Pilates experience', from brand new to trained teacher. Anyone who isn't brand new is asked where and when their last class was. Clients tick the sessions that interest them (mat, reformer, private, duet, pregnancy or postnatal, online) and the goals they care about, such as core strength, posture or getting back to exercise after a break, with space to say more.

Page two is health. A current or recent injury asks for details; a checklist covers osteoporosis, blood pressure, disc problems, joint replacements, recent surgery, hernia and pelvic floor concerns. The pregnancy question branches: pregnant clients give their trimester and due date, postnatal clients give the weeks since birth, the type of birth and anything they've noticed since. The studio policy and consents close the form, and the signed intake is filed as a PDF in your FileIt vault.

The pilates studio intake form, page by page

1 You & your goals

Full name, date of birth, mobile and email. 'Your Pilates experience' is required; everyone except complete beginners is asked 'Where and when was your last class?'. Required checklists for 'Which sessions interest you?' and 'What would you like Pilates to help with?' (with an Other option), then a box to say more, other exercise each week and how they heard about you.

2 Health & studio policy

A required question about injury or pain in the last 12 months, with details on a yes. A required conditions checklist — details asked unless 'None of these' is ticked. 'Are you pregnant, or have you given birth in the last 12 months?' reveals trimester, due date and anything a midwife or doctor has said for pregnancy, or weeks since birth, type of birth, postnatal check-up and concerns for postnatal clients. Then your studio policy placeholder, required consents for health information and policy, an optional news consent and the signature.

Make the template yours

  • Replace the policy placeholder with your booking, cancellation, grip-sock and refund rules
  • Edit 'Which sessions interest you?' to list the classes on your timetable
  • Add your reformer weight or height limits as help text if your equipment has them
  • Remove the pregnancy branch if you don't teach pre- or postnatal clients
  • Add a question about preferred teacher if you have several on staff
  • File intakes in a vault folder per teacher or per studio location

Tips for a better pilates studio intake form

  • Read the health page before the first session, not at the door
  • Ask returning clients to fill in a fresh intake after a pregnancy, surgery or long break
  • Keep the goals in mind when you write a client's programme and refer back to them later
  • Decide which teachers can open the PDFs, since they hold health details
  • Use the Responses table filters to find everyone who ticked reformer before you schedule extra sessions

Every response becomes a PDF in your vault

The signed intake is saved as a PDF in your FileIt vault and you can be emailed when it arrives. The client sees your confirmation message and can receive a copy.

Search the Responses table by name before a session to read a client's goals and health notes, or export to CSV to plan a new course.

  1. Start from this template. It opens in the FileIt Forms designer — change any question, add pages, set the rules for when questions appear.
  2. Share it. Turn on a public link, or send it to people by email, each with their own link. They don’t need a FileIt account.
  3. Get the answers as PDFs. Each response is saved as a PDF in the vault folder you choose, with uploaded files attached — and listed in a Responses table you can export to CSV.

Pilates studio intake form: frequently asked questions

Is this Pilates intake template free?

Yes. It's a starter template in the FileIt Forms app, available on the free plan, and every question is editable.

Does the form decide whether a client can do Pilates?

No. It only collects what the client tells you. Your teachers, and the client's own health professionals, make those calls.

What do pregnant clients see?

Choosing 'I'm pregnant' adds the trimester, due date and whether a midwife or doctor has said anything about exercise.

What about clients who've recently given birth?

They're asked how many weeks since the birth, the type of birth, whether they've had a postnatal check-up and anything they've noticed since.

Can I add my own studio terms?

Yes. Replace the placeholder paragraph with your policy; clients must tick that they agree before they can sign.

Do clients need a FileIt account?

No. They complete and sign it from a link.
