What is the pet travel checklist form template?

Travelling across borders with a dog or cat can involve a microchip, a rabies vaccination given in the right order, waiting periods, blood tests, tapeworm treatment, a health certificate from an authorised vet and a confirmed pet place with the airline or ferry. Missing a single step can mean being turned away at check-in.

This checklist helps owners plan it all: the trip and the pet, the destination's rules and where they were checked, microchip and treatment dates, a status grid for every document, and the booking and carrier details. Date questions for blood tests, tapeworm treatment and health certificates appear only if the owner ticks that the destination needs them.

It doesn't tell you the rules for a particular country, because they vary and change. It gives you a structured way to record what the official source says and track each step. Each checklist is saved as a PDF in FileIt with copies of your documents attached.

The pet travel checklist form, page by page

1 Trip and pet

Record the owner's name and email, the pet's name, species, breed, weight with carrier (in kg or lb) and date of birth, where you're travelling from and to, the departure and return dates, and how you're travelling: in the cabin, in the hold or as cargo, by ferry, train, car or with a pet transport company.

2 Destination rules and vet

Note where you checked the entry rules and when, then tick what the destination requires, from a microchip and rabies vaccination to a blood test, tapeworm treatment, a health certificate, import permit, quarantine or an arrival notice. If your pet could be a restricted breed there, the form asks what you found out. Add your vet practice and appointment date.

3 Health and documents

Record whether your pet is microchipped, with the number and implant date, and the rabies vaccination date and expiry. Dates for the blood test, tapeworm treatment and health certificate appear when you've ticked them as required. A grid marks each document as ready, in progress or not needed, and you can upload up to ten document copies.

4 Booking and carrier

Add the airline, ferry or transport company, the pet booking reference and whether the pet's place is confirmed. Tick off the carrier and kit list, and note medication and feeding, a vet at the destination and anything else.

Make the template yours

  • Add the specific requirements for a destination you send clients to often
  • Remove the travel modes you don't offer if you're a pet transport business
  • Collect the owner's phone and address if you'll arrange collection
  • Notify your coordinator by email when a client submits their checklist
  • Make the email field required so you can send each client their copy
  • File each client's checklist into their own vault folder

Tips for a better pet travel checklist form

  • Start planning months ahead: some destinations require a waiting period after a rabies vaccination or blood test
  • Check the destination's rules on its official government website and again just before you travel
  • Make sure the microchip is implanted before the rabies vaccination if your destination requires that order
  • Confirm your pet's place with the airline or ferry separately from your own booking
  • Get your pet used to the carrier well before the trip

Every response becomes a PDF in your vault

The checklist is saved as a PDF in your FileIt vault with any documents you uploaded, and you can receive a copy by email.

Because FileIt is a document vault, you can also keep your pet's passport and vaccination record there and set a reminder for when the rabies vaccination needs renewing.

  1. Start from this template. It opens in the FileIt Forms designer — change any question, add pages, set the rules for when questions appear.
  2. Share it. Turn on a public link, or send it to people by email, each with their own link. They don’t need a FileIt account.
  3. Get the answers as PDFs. Each response is saved as a PDF in the vault folder you choose, with uploaded files attached — and listed in a Responses table you can export to CSV.
