What is the noise or neighbour complaint form template?
Noise complaints are hard to act on when they arrive as an angry late-night message with no dates or details. This template asks residents for the facts: what the problem is, where it comes from, when it started, how often it happens and at what times, the most recent occurrence, and a description of what they saw and heard. They can attach photos, screenshots or a noise diary.
It covers more than noise — smoke or smells, parking, rubbish, pets and anti-social behaviour — and asks noise-specific questions only when noise is the issue. The last page records the impact on a 1–5 scale and in what way, whether the resident has spoken to the person responsible (including the option that they don't feel safe to), whether it's been reported elsewhere, and what they'd like to happen.

1 Your details
A note at the top points anyone in danger to the emergency services. Then name, unit or address, phone and email, and whether the resident would like their name kept from the other party — with a note that this can't always be guaranteed if things escalate.

What's the problem? — noise, smoke or smells, parking, rubbish, pets, anti-social behaviour or other. Noise asks for the Type of noise. Then where it's coming from, when it started, How often does it happen?, Usual times, the Most recent occurrence and roughly when, a description, and uploads for photos, screenshots or a noise diary.

How much is this affecting you? on a 1–5 scale and in what way, whether they've spoken to the person responsible (a yes asks what happened), where else it's been reported, what they'd like to happen, and a confirmation that the report is true and may be shared with the people who need to deal with it.

Tips for a better noise or neighbour complaint form
- Encourage residents to keep a diary with dates and times — a pattern is more useful than a single incident
- Don't reveal who complained unless you have to, and say so honestly if it may come out
- Hear the other side before deciding anything
- Reply to every report, even if only to say what you'll do next
- Point residents to the police or emergency services for anything threatening or dangerous

Can residents complain anonymously?
They give their name, but can ask for it to be kept from the other party. The form explains that this can't always be guaranteed.
Can they attach evidence?
Yes. Up to five files — photos, PDFs, Word or text documents such as a noise diary. Audio and video files can't be uploaded, so ask for recordings separately if you need them.
Does it cover problems other than noise?
Yes — smoke or smells, parking, rubbish, pets, anti-social behaviour and anything else. Noise-specific questions only appear for noise.
Is this form for emergencies?
No. A note at the top of the form tells residents to contact the emergency services first if anyone is in danger, and the confirmation message repeats it.
