What is the nail service client card form template?
Nail technicians need to know a few things before they start: whether there's an infection or broken skin, any allergy to acrylates or glue, and what the client likes. Paper record cards get lost or go out of date, and allergies are the detail that matters most.
This template covers it over four pages. Clients give their contact details and whether it's their first visit. The health page asks about current conditions, from fungal infections to diabetes, allergies and reactions (a previous reaction to gel polish asks for details), medication and how strong their natural nails are. The preferences page records usual services, shape, length, colours, inspiration photos and how hard they are on their hands. The last page has aftercare advice, two acknowledgements, photo and reminder preferences, a signature and a notes box for the technician.
Each card is saved as a PDF in your FileIt vault, ready to check before the next appointment.

Usual services, from manicure to BIAB, acrylic or gel extensions and nail art, preferred shape and length, favourite colours, up to three inspiration photos, and how much the client uses their hands.
