What is the lash extension consultation form template?

Lash adhesive sits right next to the eye, so lash artists need to know about recent infections, eye surgery, dry eye and any history of reactions before they start. A consultation form also records the client's choice on a patch test, which matters if a reaction happens later.

This template has four pages. Clients give contact details, date of birth, appointment date and whether they've had extensions before. The health page covers eye conditions, contact lenses, allergies, previous reactions to lashes, lifts or tints, pregnancy and sleeping position, with notes when something needs discussing. The look page records style, eye shape effect, length, curl and an inspiration photo. The last page records the patch test (declining requires a separate acknowledgement), explains what to expect and the aftercare, then takes consent, photo permission, a signature and the artist's lash map notes.

Each consultation is saved as a signed PDF in your FileIt vault.

The lash extension consultation form, page by page

1 About you

Client name, phone, date of birth and appointment date are required, with optional email. Whether they've had extensions before; if so, whether they're wearing a set now and who applied it.

2 Eye health & allergies

A required checklist of eye and health conditions; anything other than 'None of these' shows a note that treatment may be adapted or postponed. Contact lenses (a yes shows a note to remove them), allergies including cyanoacrylate, latex and tape, and any previous reaction, which asks what happened. Optional pregnancy and sleeping position questions.

3 The look you want

A required style from classic to mega volume, plus optional look, length, curl and up to two inspiration photos.

4 Patch test & consent

Whether the client has had a patch test, wants one first, or declines; declining needs a separate acknowledgement and 'done' asks for the date. Paragraphs explain what to expect and the aftercare. Required treatment consent and aftercare acknowledgement, photo permission, signature and artist notes.

Make the template yours

  • Replace the risks and aftercare paragraphs with your own wording
  • Change the patch test timing to match your insurer's requirements
  • Add your infill prices to the aftercare paragraph
  • Add lash lift or tint questions if you offer them
  • Ask returning clients to update the form at least once a year
  • Remove the sleep question if you don't use it for lash mapping

Tips for a better lash extension consultation form

  • Check your insurer's patch test rules before you publish the form
  • Read the reaction answers before the client lies down
  • Keep a record of the adhesive batch used in the artist notes
  • Send the form with the booking confirmation so it's done before arrival
  • Have a qualified professional review the health questions

Yes. The aftercare and risks text are paragraphs you can rewrite in the designer.

Is the consent wording legally sufficient?

FileIt doesn't give legal advice. Check the wording with your insurer or adviser.
