What is the municipal fee waiver request form template?

Councils and cities often have discretion to waive or reduce fees for people on low incomes, nonprofits, community events and billing errors, but requests arrive in emails and letters with half the facts. This form asks which fee, how much, what's being asked for and why, and takes the supporting documents.

Logic follows the request. A partial reduction asks how much the applicant can pay, a payment plan asks what instalments they could manage, and more time asks for a date. Reasons open their own questions: which benefit, household size and income for low income, a charity or tax-exempt number for nonprofits, and who benefits for community events. Organisations are asked their name.

A note says that sending the request doesn't pause the fee unless you confirm it in writing. Each signed request is saved as a PDF in FileIt.

The municipal fee waiver request form, page by page

1 About you

Name, "Applying as" (individual or household, nonprofit, community group, small business or school), organisation name if it isn't an individual, email, phone and address.

2 The fee

"What is the fee for?" covers facility or park hire, event permits, building permits, utility bills, parking charges, bulky waste, recreation programmes and licences. The account or invoice number, "Amount of the fee", due date and "What are you asking for?": a full waiver, a partial reduction, a payment plan or more time to pay, each with its own follow-up.

3 Reason & eligibility

"Why are you asking?" offers low income, a public benefit, senior citizen, disability, nonprofit use, community benefit, a billing error and hardship. A public benefit asks which one, low income asks for household size and yearly income, nonprofit use asks for the charity or tax-exempt number, and community benefit asks who benefits and how. "Explain your request" and "Have you had a fee waived or reduced by us before?" follow.

4 Documents & declaration

"Supporting documents" takes up to ten files. A note says the fee isn't paused unless you confirm in writing, then a declaration and a signature.

Make the template yours

  • List only the fees your policy allows to be waived
  • Put your eligibility rules or income limits in the cover text
  • Change the currency on the amount questions
  • Make supporting documents required if your policy always needs evidence
  • Notify the department that owns each fee

Tips for a better municipal fee waiver request form

  • Check the due date first so requests are decided before the fee falls due
  • Apply your policy the same way to similar requests
  • Ask for evidence that's missing rather than refusing outright
  • Record the decision and reason with the request PDF
  • Export requests to CSV to report on waivers granted each year

Every response becomes a PDF in your vault

Each signed request is saved as a PDF in the folder you choose, with documents attached. Your team is emailed, and the applicant gets a copy if you leave that on.

The Responses table lists requests by fee type and amount, and the CSV export gives you a register of waivers for audit and reporting.
