What is the business license application form template?
Licensing staff need the same core facts for most applications: the business's legal name and structure, who owns it, where it trades, what it does, and the documents that prove it. This form asks for them across four pages and ends with a declaration and signature.
It adapts to the application. Renewals and changes ask for the current licence number. Home-based businesses see a note that zoning or planning approval may also be needed. Leased premises ask for the landlord. Food and drink businesses see a note that a health permit is usually needed too.
Each signed application is saved as a PDF in FileIt with the uploaded documents attached.

The business license application form, page by page
1 The business
"Type of application" (new, renewal, change of ownership or change of address) asks for the current licence number unless it's new, plus the previous owner for a change of ownership or the previous address for a move. Then legal and trading names, "Business structure", registration and tax numbers, the date the business opened or will open, phone and email.
2 Owners
The owner or principal officer's name, title, phone, email and home address, and "Other owners, partners or officers" with name, title and share for each.
3 Premises & activities
"Business address" and "Type of premises" (commercial unit, home, mobile or market stall), with the zoning note for home businesses. "Do you own or lease the premises?" asks for the landlord if leased. "What does the business do?", "Which apply?" (retail, food or drink, alcohol, beauty and tattoo, professional services, trades, childcare, entertainment, manufacturing), opening hours, employees, new signs and other permits.
4 Documents & declaration
"Supporting documents" takes up to ten files, such as a registration certificate, lease, owner ID and insurance. "Does the business have liability insurance?", a declaration and a signature finish the application.

Every response becomes a PDF in your vault
Each signed application is saved as a PDF in the folder you choose, with documents attached. Your licensing team is emailed, and the applicant gets a copy if you leave that on.
The Responses table lists applications with search by business name or address, and the CSV export can feed your licence register.
- Start from this template. It opens in the FileIt Forms designer — change any question, add pages, set the rules for when questions appear.

- Get the answers as PDFs. Each response is saved as a PDF in the vault folder you choose, with uploaded files attached — and listed in a Responses table you can export to CSV.
