What is the mold inspection report form template?

A mold inspection is only as useful as the report that comes out of it. Clients, landlords, remediation contractors and sometimes insurers will all read it, and each of them wants the same answers: where is it, how big is it, where is the water coming from, and what should be done.

This template covers the inspection in two pages. The first records the property, why the inspection was requested, the areas checked, temperature and humidity, moisture readings and whether there's visible growth — with its location and approximate size when there is. The second handles samples, the likely moisture source and your recommendations. Answer yes to "Samples taken?" and the lab section appears: sample types, count, laboratory, chain-of-custody number, dates and a place for the lab report.

Each report is saved as a PDF in your FileIt vault with photos attached, and the client can get a copy by email. When the lab results arrive later, file them alongside the report in the same folder.

The mold inspection report form, page by page

1 Property & findings

The Inspector, Company, Inspection date, Client name, Client email and Property address open the report, with Property type alongside. "Why was the inspection requested?" covers visible growth, a musty smell, a recent leak or flood, occupant concerns, buying or leasing, or a check after remediation. Areas inspected runs from basement and crawl space to attic, bathrooms, laundry and the HVAC system.

Indoor temperature, Indoor relative humidity (%), Outdoor relative humidity (%) and the Moisture meter used come next, then a Moisture readings box set out as location, material and reading. "Visible mold growth?" is required; yes opens "Where, and what it looks like" and the Largest single affected area.

2 Samples, cause & remediation

"Samples taken?" opens Sample types (spore trap air, outdoor comparison air, swab, tape lift or bulk material), Number of samples, Laboratory, Chain-of-custody number, the date Sent to the lab, Results expected and an optional Lab report upload.

Likely moisture source lists plumbing and roof leaks, condensation, poor ventilation, foundation seepage, flooding, HVAC condensate or not yet determined. The inspector picks a Suggested next step, writes Remediation recommendations, says whether a check after remediation is recommended, adds photos and signs. A short note on the PDF says the report reflects conditions on the inspection date.

Make the template yours

  • Switch the temperature label to °C if you work in metric
  • Add the sampling methods or labs you use as fixed options
  • Add a fee field, or an invoice number, for jobs billed on the day
  • Reword the closing note to match your company's standard report wording
  • Copy a remediation partner on every report through the notification emails
  • File by property address so follow-up inspections land beside the first

Tips for a better mold inspection report form

  • Always take an outdoor humidity reading — it gives the indoor numbers meaning
  • Photograph the meter display next to the material you're testing
  • Describe growth by location and size, and leave identification to the lab
  • Write the chain-of-custody number on the report before the samples leave your hands
  • Find the water first — recommendations that skip the source tend to fail
  • Upload the lab report to the same vault folder as soon as it arrives

Every response becomes a PDF in your vault

The report is filed in your FileIt vault as a PDF named with the date and client, with photos and the lab report if you attached it. The client gets a copy if their email was entered, and the account owner is notified.

Many inspectors use the Responses table to track which jobs are still waiting on lab results, and export recommendations to CSV when they quote for remediation.

  1. Start from this template. It opens in the FileIt Forms designer — change any question, add pages, set the rules for when questions appear.
  2. Share it. Turn on a public link, or send it to people by email, each with their own link. They don’t need a FileIt account.
  3. Get the answers as PDFs. Each response is saved as a PDF in the vault folder you choose, with uploaded files attached — and listed in a Responses table you can export to CSV.
