What is the asbestos survey report form template?

An asbestos survey is only useful if the people who work on the building can read and trust it. That means saying clearly where the surveyor went, where they couldn't go, what they found in each place and what should happen next.

This three-page report starts with the surveyor and company, the client, the property address, building type and approximate year built, and the survey type: management, refurbishment, demolition, a re-inspection of known materials or a sampling visit. A refurbishment survey asks which planned works it covers. Areas surveyed is required, and Areas not accessed, and why carries a reminder to treat anything in those areas as containing asbestos until it's checked.

Page two records up to three materials, each with location, material type, extent, condition and sample reference, plus a full material register upload for bigger buildings. Page three covers the samples, the laboratory and the results. When asbestos is detected, the form asks for the positive sample references, the type identified, the highest risk, recommendations and a re-inspection date. The surveyor signs, and the report is filed as a PDF in your FileIt vault.

The asbestos survey report form, page by page

1 Property & survey

Surveyor, Survey date, Client and Property address are required, with company, accreditation or licence number, report reference, client email, building type and year built. Survey type has five options; choosing a refurbishment survey asks for the planned works it covers. Areas surveyed is required, and Areas not accessed, and why and whether the building was occupied are optional.

2 Materials

Material 1 asks for Location, Material (from pipe lagging, sprayed coatings and insulating board to ceiling tiles, textured coatings, cement sheet, floor tiles and gaskets), Extent, Condition (good, minor damage, or poor or damaged) and Sample reference, or 'presumed'. Materials 2 and 3 start with only their location; the other questions appear once one is entered. A full material register and photos can be uploaded.

3 Results & recommendations

Samples taken and Laboratory come first, then Sample results: asbestos detected, none detected, awaiting results, or no samples with materials presumed. A positive result opens Positive sample references, Asbestos type identified, Highest risk assessed, Recommendations (from leaving in place and managing to removal before works, restricting access or re-inspecting), the recommendation details and a re-inspection date. The laboratory certificate, a summary, the declaration and the surveyor's signature finish the report.

Make the template yours

  • Add more material blocks for buildings that usually have more than three
  • Swap the risk choices for the scoring system your firm uses
  • Add the duty holder or building manager as a second signature
  • File reports into a vault folder per building or per client
  • Put your firm's limitations wording in the cover text

Tips for a better asbestos survey report form

  • Photograph every material with its sample label visible
  • Be specific about areas not accessed; vague limitations cause problems later
  • Submit once the lab results are back, or mark them as awaiting
  • Give each recommendation a timescale
  • Share the report with anyone planning work in the building
