What is the franchise location audit form template?
A franchise brand is only as consistent as its weakest location. Regular audits are how franchisors check that signage, service, product and cleanliness meet the standard customers expect — but audits done on paper or from memory are hard to compare and easy to dispute.
This franchise location audit gives every visit the same structure. The first page records the auditor, location, franchisee, address, visit date and time, whether the visit was announced, unannounced or a follow-up, the manager on duty and the result of the last audit. The second page rates six areas — exterior, interior, staff and service, product, cleanliness, and compliance and records — on grids of meets standard, needs improvement, fails or not applicable, with an optional overall score.
The final page records the overall result, any critical issues and what was done about them, other findings, what the location does well, photos, and an action plan with owners and dates. The auditor signs, and so does the franchisee or manager when the plan has been discussed. Each audit is saved as a signed PDF in your FileIt vault.

The franchise location audit form, page by page
1 Location & visit
The auditor records their name, the location name or number, franchisee, address, visit date and arrival time. Type of visit — announced, unannounced or follow-up — is required, alongside the manager on duty and the result of the last audit, or first audit.
2 Brand standards
Six required grids rate each item as meets standard, needs improvement, fails or not applicable. Exterior covers signage, entrance and parking, windows and promotional material; Interior covers layout, fixtures, lighting, atmosphere and restrooms; Staff and service covers uniforms, customer service, product knowledge and staffing levels.
Product checks the approved range, approved suppliers, presentation or portion standards, pricing and stock rotation. Cleanliness covers front of house, back of house, storage and waste areas, and Compliance and records checks licences, safety signage, fire exits, food safety records and training records. Brands that score audits can enter an overall percentage.
3 Findings & action plan
Overall result is pass, pass with actions or fail. Any critical issue needing immediate action — such as a food safety risk or blocked fire exit — must be described along with what was done on the spot.
Other findings, what the location is doing well and up to 10 photos follow. The action plan is required, with an owner and due date for each item, and a follow-up visit date. If the plan was discussed with the franchisee or manager, their comments and signature are captured alongside the auditor's.
Make the template yours

- Remove the food safety row for non-food franchises
- Add a section for marketing or local promotions compliance
- Change the rating columns to a points scale if you score audits
- File each location's audits in its own vault folder
- Send owner notifications to the franchise operations team
Tips for a better franchise location audit form
- Use the same checklist on every visit so results can be compared over time
- Photograph anything rated fails, so there's no dispute about what was seen
- Record what the location does well as well as what needs fixing
- Agree realistic due dates in the action plan with the franchisee, not for them
- Deal with critical issues before leaving the site, and record what was done
- Check the previous action plan at the start of each follow-up visit
Every response becomes a PDF in your vault
Each audit is saved as a signed PDF in the vault folder you choose, with photos attached, giving each location a dated audit history. Owner notifications email the operations team when an audit is submitted.
The Responses table lists every audit, so you can see results across the network, find locations with critical issues and export to CSV to track trends.
- Start from this template. It opens in the FileIt Forms designer — change any question, add pages, set the rules for when questions appear.

- Get the answers as PDFs. Each response is saved as a PDF in the vault folder you choose, with uploaded files attached — and listed in a Responses table you can export to CSV.
Franchise location audit form: frequently asked questions
Is this template free?
Yes. It's included in the FileIt Forms app on every account, including the free plan, and every question can be edited, removed or added to.
Can the auditor complete it on a phone or tablet?
Yes. The form works in a mobile browser, so auditors can fill it in and take photos as they walk the location.
Can the franchisee sign the audit?
Yes. When the action plan has been discussed with the franchisee or manager, a second signature field appears for them.
Does it calculate a score automatically?
No. The grids record a rating per item. If your brand scores audits, the auditor enters the overall percentage, or you can export the ratings to CSV to calculate your own.
Can I use my own brand standards?
Yes. Edit the grid rows in the designer to match your standards manual, and add or remove whole sections.
How many photos can be attached?
Up to 10 per audit, each up to 10 MB.
Does it cover legal or food safety compliance?
It records whether the items you list are in place. It isn't a substitute for the inspections or records required by the authorities where the location operates.
Does the auditor need a FileIt account?
No. Anyone with the form's link can fill it in without creating a FileIt account.
