What is the franchise inquiry form template?
Franchise development teams get a lot of casual enquiries alongside the serious ones. Without knowing a prospect's preferred territory, how much capital they have access to and how involved they plan to be, it's hard to decide who to call first and what to talk about.
This franchise inquiry form asks for contact details and ownership structure, preferred and second-choice territories, whether a site is in mind, how many units and when, planned involvement, liquid capital and total investment ranges, funding sources, current occupation, previous business ownership and relevant experience — plus why they're interested and any questions for you.
Every inquiry is saved as a PDF in your FileIt vault, so you have a tidy record of each lead to review before the first call and to refer back to through the discovery process.

The franchise inquiry form, page by page
1 About you
The prospect gives their name, email, phone and home address, the best time to call and how they heard about you, and whether they'd own the franchise alone, with a partner, or as part of an investor group or company — naming partners where relevant.
2 Territory & timing
They name a preferred territory and a second choice, say whether they have a site in mind (with a box for the address if they do), how many units they're interested in, when they'd like to open and whether they'd be an owner-operator, semi-absentee or investor only.
3 Investment
Two dropdowns capture liquid capital available and the total investment they're considering, and a checklist shows how they'd fund it. The page explains the answers are confidential and this isn't a credit application.
4 Experience & motivation
The prospect gives their current occupation and employer, whether they've owned a business before (with details if so), ticks relevant experience, lists other franchises they're considering, explains why they're interested and adds any questions. They agree to be contacted and confirm this is an expression of interest only.
Make the template yours
- Change the investment bands and currency to match your franchise's actual investment range
- Replace the free-text territory with a dropdown of the regions you're offering
- Remove the multi-unit option if you only offer single units
- Add a question about a minimum net worth if your system requires one
- Notify your franchise development manager directly with the notification email setting
- Point the confirmation message to your discovery day or information pack
Tips for a better franchise inquiry form
- Call new inquiries quickly — prospects often contact several brands at once
- Use liquid capital and timeframe together to prioritise follow-ups
- Treat the investment answers as confidential and share them only with the people who need them
- Follow the franchise disclosure rules that apply where you operate before discussing terms in detail
- Keep the form short enough for a first contact; save detailed financials for the formal application
Every response becomes a PDF in your vault
When a prospect submits the form, FileIt saves it as a PDF, files it in your vault and emails you straight away. The prospect can get a copy of what they told you.
The Responses table lists every inquiry with territory, timeframe and investment range, which you can sort or export to CSV to plan calls and see where demand is coming from.
- Start from this template. It opens in the FileIt Forms designer — change any question, add pages, set the rules for when questions appear.
- Share it. Turn on a public link, or send it to people by email, each with their own link. They don’t need a FileIt account.
- Get the answers as PDFs. Each response is saved as a PDF in the vault folder you choose, with uploaded files attached — and listed in a Responses table you can export to CSV.
