What is the book club registration form template?
A book club lasts when its members agree on two things: what to read and when to meet. Getting there usually takes a long thread of replies. This short registration form asks every new member the same questions once, so the organiser can see the overlap at a glance and suggest a first book that most people will actually finish.
New members give their name, email, an optional phone number for last-minute changes, their town or neighbourhood and how they heard about the club. They tick 'Genres you enjoy' from eleven choices (or add their own), say what they'd rather not read, name a book they loved recently and describe how much they read. Then come the logistics: 'Meeting format' (in person, online, or either), the days that suit them and the time of day.
Anyone who could meet in person is asked 'Would you be willing to host a meeting at your home?'. Members who say yes or maybe can say how often they'd host and note things guests should know, like pets, stairs or parking. The form ends with access needs, an offer to choose a book one month and a consent to receive club emails. Each registration is filed as a PDF in your FileIt vault.
