What is Alabama Form A4?
Alabama Form A4 (Rev. 4/2025) is the certificate every employee gives an employer, on or before starting work, to set Alabama income tax withholding. You claim one personal exemption: none (0), Single (S) or Married Filing Separately (MS) at $1,500, or Married (M) or Head of Family (H) at $3,000. Then you add the number of dependents you support and any extra amount per pay period.
The form warns that the exemptions may never exceed what you are entitled to, and that inflating them carries a $500 penalty under Section 40-29-75. Part II is completed by the employer, who keeps the certificate on file.

1 Your details

2 Lines 1 to 3: your personal exemption
Choose one. Line 1: write 0 to claim no personal exemption and withhold at the highest rate. Line 2: S for the $1,500 single exemption or MS for married filing separately. 3 Line 4: dependents
Enter how many dependents, other than your spouse, you will provide more than half the support for during the year. They must be related to you as the form lists: children, stepchildren, grandchildren and in-laws; parents, grandparents and step-parents; brothers, sisters and half- or step-siblings; or an uncle, aunt, nephew or niece related by blood.
4 Line 5: additional amount
Optionally, enter an extra amount to deduct each pay period.
5 Sign and date
Certify under penalties of perjury that the certificate is true, correct and complete, and sign and date it. You can sign by hand or with FileIt eSignature.
6 Employer: line 6 and Part II
The employer enters the total exemptions on line 6, which is the withholding table column to use. The form's example is M on line 3 and 2 on line 4, giving column M-2. The employer then adds its name, EIN and address.
Common mistakes to avoid
- Claiming more exemptions than you are entitled to, which carries a $500 penalty.
- Writing a letter on both line 2 and line 3. You claim one personal exemption.
- Counting your spouse as a dependent on line 4. Line 4 is for dependents other than your spouse.
- Counting someone who doesn't get more than half their support from you, or who isn't related to you as the form lists.
After you fill it out

What do S, MS, M and H mean on Form A4?
S is the single exemption and MS married filing separately (each $1,500). M covers yourself and your spouse and H is single claiming head of family (each $3,000). Writing 0 claims no personal exemption.
What is line 6 for?
The employer completes it with the withholding table column to use, such as M-2 for a married employee with two dependents.
What if I claim too many exemptions?
The form says an employee who inflates the number of exemptions pays a $500 penalty under Section 40-29-75, and the employer must withhold at the highest rate until a corrected A4 is filed.
Isn't Form A-4 Arizona's?
Arizona also has a Form A-4, its withholding election, which FileIt lists as US A-4. Alabama's certificate is listed as AL-A-4.
