SGK-020 — Genel Sağlık Sigortası Giriş Bildirgesi (General Health Insurance Entry Notification)

What is SGK-020 general health insurance enrolment?

The Genel Sağlık Sigortası Giriş Bildirgesi (general health insurance entry notification, listed by SGK as SGK-020) tells the Sosyal Güvenlik Kurumu (SGK) that a person has come within general health insurance under article 60 of Law 5510 on Social Insurance and General Health Insurance. It is used for people who are not insured through work. Examples include foreign residents with a residence permit, stateless people and refugees, people receiving unemployment benefit, orphans, village guards and people whose cover depends on household income.

The explanations on the back of the form say it is normally given to SGK online or electronically. The paper version is used only where the electronic route is not yet available. In that case the insured person signs it (section 19) and it must reach SGK within one month. Depending on the status, either the person files it themselves or a responsible body does. For example, İŞKUR files for unemployment-benefit recipients, the police for stateless people and refugees, and the Governor's office for village guards.

Who needs to fill out SGK-020 general health insurance enrolment?

• Foreign nationals living in Türkiye with a residence permit who are not otherwise insured (status 60/d).
• People not covered by any other clause of article 60 and not covered by another country's health insurance (60/g).
• People whose general health insurance premium depends on household income per person compared with the minimum wage (the 60/g income bands).
• People whose household income per person is below one third of the minimum wage (60/c-1).
• Stateless people and refugees, orphans, people covered by Law 2828, village guards and unemployment-benefit recipients. For these groups the responsible institution usually files the notification.

When to use SGK-020 general health insurance enrolment

• When you first come under general health insurance and are not insured through employment. Tick 'İlk' (first) at the top.
• When you come back under general health insurance after a period covered some other way. Tick 'Tekrar' (again).
• When the online or electronic route is not available to you and SGK accepts the paper form.
• Within one month of the date general health insurance starts, as the form's explanations require.

What you need before you start

• Your T.C. kimlik number, or for foreign nationals the 11-digit number used as your social security number.
• Your identity card (nüfus cüzdanı) or population register extract for your parents' names, place of birth and registration details (il, ilçe, mahalle/köy, cilt, aile and birey sıra no).
• Your residence permit and nationality if you are a foreign national.
• Your full home address with neighbourhood, district, province and postcode.
• For men, your military service status and service dates.
• Which article 60 status applies to you, and the date general health insurance starts.
• If you worked before 1 October 2008, which fund you were with (SSK, Bağ-Kur, Emekli Sandığı or a fund under provisional article 20 of Law 506).

How to fill out SGK-020 general health insurance enrolment, step by step

1 Social security number and type of notification (Sosyal güvenlik sicil numarası, Belgenin mahiyeti)

Write your T.C. kimlik number in the boxes at the top. According to the explanations, foreign nationals use the social security number SGK gives them. The form has 13 boxes and the 11 digits go in from the left.

Next to it, tick 'İlk' if you have never had general health insurance before, or 'Tekrar' if you have.

2 A — Identity details, items 1 to 10 (Kimlik bilgileri)

First name, surname, surname at birth, father's and mother's first names, place and date of birth, sex and marital status. The form offers only Evli (married) or Bekar (single). Item 10 is where you are registered in the population register: province, district, neighbourhood or village, volume number and the family and individual serial numbers. These come from your identity card or the population register extract.

FileIt prints names exactly as you type them. It does not convert them to capitals, so Turkish letters such as İ and ı stay correct.

3 Items 11 to 14: nationality, education, military service and address

Item 11 is your country if you are a foreign national. Item 12 is your highest level of education: İlköğretim (primary), Lise (high school), Lisans (university degree), Lisansüstü (postgraduate), Okuryazar (literate) or Diğer (other). Item 13 applies to men only: Er (served as a private), Yedek Sb (reserve officer), Muaf (exempt) or Tecilli (deferred), with service start and end dates for those who served. Item 14 is your home address, laid out as boulevard, avenue, street, building and flat number, neighbourhood or village, postcode, district and province.

4 B — Item 16: Status of the insured (Sigortalının statüsü)

Tick the one article 60 clause that applies to you. The left column lists 60/c-1 (household income per person below one third of the minimum wage), 60/c-2 (stateless people and refugees), 60/c-7 (people covered by Law 2828, and orphans), 60/c-9 (village guards), 60/d (foreigners with a residence permit) and 60/e (unemployment-benefit recipients). The right column lists the 60/g groups: three household-income bands and people not covered by any other clause or by another country's health insurance.

5 Items 17 and 18: work before October 2008 and start date

If you worked before 1 October 2008, item 17 records which fund you were with: SSK, Bağ-Kur, Emekli Sandığı or a fund under provisional article 20 of Law 506. FileIt marks an X under the one you choose. Item 18 is the date your general health insurance starts.

6 C — Declaration, item 19 (Beyan ve taahhütler)

You declare that the information is true and that you will tell SGK straight away about any change. On a paper form you sign it by hand. FileIt prints the date and your name beside 'Sigortalının Adı-Soyadı'.

7 Items 20 and 21: the notifying institution

Only when an institution is obliged to notify SGK for you, for example İŞKUR or a Governor's office. It writes its 26-digit workplace registration number (işyeri sicil numarası), its name and address, then an authorised official signs and stamps it. The explanations say items 20 and 21 stay blank when the insured person fills in and hands in the form themselves. In FileIt this is an optional part.

Common mistakes to avoid

• Using the paper form when SGK expects the notification online. The explanations say paper is only for when the electronic route isn't available.
• Missing the one-month deadline stated in the explanations.
• Ticking more than one status in item 16, or a status that doesn't match your situation.
• Writing a T.C. kimlik number with a typo. FileIt checks the check digits.
• Filling in items 20 and 21 when you are filing the form yourself. Leave them blank.
• Forgetting to sign item 19 on the printed form.
• Upper-casing Turkish names with the wrong İ or I. Type your names exactly as on your ID.

After you fill it out

Print the completed form from your FileIt vault and sign item 19 by hand under 'İmzası'. If an institution is filing for you, its authorised official dates, signs and stamps item 21.

Give the form to SGK yourself, or through the institution responsible for your status, within one month. FileIt does not submit the form, and SGK normally expects this notification online, so check with SGK first whether it will accept it on paper.

Keep the signed copy in your FileIt vault with your residence permit or other documents. If your situation changes, for example you start work with an employer or your household income changes, tell SGK, as you promise in the declaration.
