What is Passport Annexure D?
Annexure D is Passport Seva's specimen declaration by an applicant's parent(s) or legal guardian for the issue of a passport to a minor. It gives the child's name and date of birth, both parents' citizenship, passport number, contact details and address, and the ARN of the child's online passport application.
By signing it, the parents or guardian solemnly declare that the child is a citizen of India who has not lost, surrendered or been deprived of that citizenship, and that they take entire responsibility for the child's expenses. They also acknowledge that if the information is incorrect, the passport may be impounded or revoked and criminal proceedings may be started under the Bharatiya Nyaya Sanhita, 2023 and the Passports Act, 1967.

Who needs to fill out Passport Annexure D?
- Parents applying together for a passport for their child under 18.
- One parent applying with the other parent's details on the form, where the Passport Office asks for Annexure D.
- A legal guardian appointed by a court, applying for a minor's passport.
- Parents renewing or reissuing a minor's passport who are asked for the parents' declaration.
When to use Passport Annexure D
- After submitting the minor's passport application online, once you have its ARN (Application Reference Number).
- Before the appointment at the Passport Seva Kendra or Post Office Passport Seva Kendra, so it can be handed in with the documents.
- When one parent's consent is missing, use Annexure C instead.
What you need before you start
- The child's full name exactly as in the application, and date of birth.
- Both parents' names, citizenship, passport numbers (if any), contact numbers, e-mail addresses and addresses.
- The ARN of the child's online passport application.
- For each signer: a passport number (preferred), or an Aadhaar, Voter ID, PAN or driving licence number.
- For a legal guardian: a copy of the court order appointing them.

How to fill out Passport Annexure D, step by step
1 The minor child
The child's name and date of birth. FileIt prints the date as DD/MM/YYYY and checks the child is under 18 on the signing date.
2 Father's and mother's details
Name, citizenship, passport number, contact number, e-mail and address for each parent. FileIt wraps the address over the three dotted lines and warns you if it is too long.
3 Items 1 to 4
Item 1 takes the ARN of the passport application. Items 2 to 4 are printed declarations: the child's Indian citizenship, responsibility for the child's expenses, and the consequences of incorrect information. The form's "I/We", "am/are" and "mother/father/parent(s)/legal guardian(s)" are left for you to strike through by hand as they apply.
4 Signatures and identity numbers
Choose who signs — both parents, one parent, or a legal guardian. Under each signature the form asks for a passport number (preferably) or an Aadhaar, Voter ID, PAN or driving licence number: pick one and it prints on its own line. A parent's passport number is copied from their details. FileIt checks Aadhaar numbers against their check digit.
5 Date and place
The date and the town where you sign. Leave the date blank if you'd rather write it in by hand.
Common mistakes to avoid
- Spelling the child's name differently from the passport application.
- Leaving out the ARN, so the declaration can't be matched to the application.
- Using Annexure D when one parent has not given consent — that situation has its own declaration, Annexure C.
- A legal guardian forgetting to attach the court order appointing them.
- Signing a scan or printing a signature: the declaration is signed by hand.
After you fill it out
Print the declaration and have each parent (or the legal guardian) sign by hand above their name. Strike out the words in items 1 to 4 that don't apply.
Take it with the minor's other documents to the passport appointment. The Passport Office may ask for more information or documents under section 5 of the Passports Act, 1967 and rule 5 of the Passports Rules, 1980 (Note 2).
