What is Passport Annexure C?

Annexure C is Passport Seva's specimen declaration by an applicant's parent or guardian for issue of a passport to a minor when one parent has not given consent. The declaring parent gives the child's and both parents' details, names the parent whose signature or consent has not been obtained, and states why — from nine reasons printed on the form, (a) to (i).

The reasons range from the other parent residing or travelling abroad, through divorce or judicial separation with or without visitation rights and pending court cases, to the other parent wilfully refusing consent or having deserted the child. Each reason lists the supporting documents to enclose, and the form asks you to put a cross mark on every reason that does not apply.

Who needs to fill out Passport Annexure C?

  • A parent whose spouse lives or is travelling abroad and can't give consent.
  • A divorced or judicially separated parent with custody of the child.
  • A parent in an ongoing divorce or separation case.
  • A parent whose former partner refuses consent, or has deserted the child and can't be found.

When to use Passport Annexure C

  • When applying for a minor's passport and the other parent's signature or consent can't be obtained.
  • After submitting the online application, once you have its ARN.
  • When both parents consent, use Annexure D instead.

What you need before you start

  • The child's full name and date of birth, and the ARN of the passport application.
  • Both parents' names and, as far as you know them, citizenship, passport numbers, contact details and addresses.
  • The supporting documents for each reason you tick — for example the other parent's passport copy and proof of residence abroad, or the court order or decree on divorce or judicial separation.
  • Your passport number (preferred), or your Aadhaar, Voter ID, PAN or driving licence number.

How to fill out Passport Annexure C, step by step

1 The child and both parents

The child's name and date of birth, then the father's and mother's name, citizenship, passport number, contact number, e-mail and address. FileIt wraps each address over the three dotted lines.

2 Who declares and whose consent is missing

Choose whether you are the father, the mother or a legal guardian, and whose signature or consent hasn't been obtained. FileIt prints that parent's name in item 2.

3 Reasons (a) to (i)

Tick every reason that applies. Each one lists its supporting documents: for (d) the form says the consent of the other parent having visitation rights is also required, and for (f) the consent of both parents or permission from the court. Printed declarations: the child is an Indian citizen and exclusively in your physical custody, you alone will defend any court case arising from the passport, and incorrect information may lead to the passport being impounded or revoked and to criminal proceedings. 5 Signature, identity number, date and place

Common mistakes to avoid

  • Not crossing out the reasons that don't apply — FileIt does this from your ticks.
  • Ticking a reason without enclosing its supporting documents.
  • Ticking (d) and forgetting that the form also requires the consent of the parent with visitation rights.
  • Using Annexure C when both parents consent — Annexure D is the declaration for that.
  • Spelling the child's name differently from the application.

Print the declaration, strike out "father/mother" where it doesn't apply, and sign it by hand.

Take it to the passport appointment with the minor's application and the supporting documents for each reason. The Passport Issuing Authority decides whether the documents are sufficient for reasons (h) and (i).
