Form 6150 (Suomessa tilapäisesti oleskelevan ulkomaalaisen rekisteröinti-ilmoitus) is the Finnish Tax Administration's (Vero's) registration form for foreigners staying in Finland temporarily. Vero's form page says you use it when you request a Finnish personal identity number (henkilötunnus) and the Tax Administration handles the request. The form serves both taxpayer registration and the Population Information System, whose data is used by the tax authorities, the police and other State and municipal authorities.
The one-page form, in Finnish and English, asks for your names (including previous ones), date of birth, sex, your personal identity code or tax identification number in your country of residence, where you were born, your native language, occupation and citizenship, your addresses in Finland and abroad, when you arrived and when your stay will end at the latest, and why your details should be recorded — working in Finland, operating a trade or profession, or another particular reason. The form says you must present documentation that substantiates these details.
