What is MR-69-V authorization or power of attorney (Revenu Québec)?
Form MR-69-V is how a person or a business tells Revenu Québec that someone else may receive their confidential information, or represent them. The person granting it can be an individual (including a deceased individual or an individual in business), a corporation, a partnership, a trust or an organization. The designated person can be an individual, such as a spouse or an adult child, or a business, such as an accounting firm.
The form offers two levels. An authorization lets the designated person consult the information and documents you choose. A power of attorney also lets them act for you: submit information and documents, ask for changes to your tax file and take part in negotiations with Revenu Québec. You choose what it covers (everything, or specific files such as personal income tax returns, the solidarity tax credit, GST/HST and QST returns or source deductions) and which years or periods.

Who needs to fill out MR-69-V authorization or power of attorney (Revenu Québec)?
- Québec taxpayers who want a spouse, adult child or other family member to be able to talk to Revenu Québec about their file.
- People who use an accountant or tax preparer and want them to access, or deal with, their Revenu Québec files.
- Businesses, trusts and partnerships giving their accounting firm or bookkeeper access to QST, source deduction or corporation income tax files.
- Liquidators of a succession, tutors, curators and mandataries acting for someone else, who sign as the legal or authorized representative.
- Anyone who wants a person to handle support payment files or shelter allowance matters, which the form says must be ticked separately.
When to use MR-69-V authorization or power of attorney (Revenu Québec)
- Before someone calls or writes to Revenu Québec about your file for you.
- When you hire a new accountant or change the person who handles your taxes.
- When a family member needs to help an ageing parent with their Québec tax matters.
- When the authorization should cover only certain files or years, which you can set out in Part 3.
- Not for a change of address: the form says to use Revenu Québec's online services or the Service québécois de changement d'adresse instead.
What you need before you start
- The identification numbers of the person granting it: SIN for an individual, and the Québec enterprise number (NEQ) or Revenu Québec identification number if there is one.
- Full name, address and telephone number of that person or business.
- The designated person's name, address and telephone number, plus their NEQ or identification number if they are a business, or the last three digits of their SIN if they are an individual.
- The designated person's professional representative number (a letter followed by 6 digits), if they have one.
- The file numbers you want to cover, such as support payment files or GST/HST and QST file numbers, if the authorization is limited to specific files.
- The years or dates the authorization should cover.

How to fill out MR-69-V authorization or power of attorney (Revenu Québec), step by step
1 Part 1: The person whose information is covered
Give the numbers that identify the person or business (NEQ, identification number, SIN), then either an individual's title, last name and first name, or the name of the business or trust, and the address and telephone number.
The form's boxes hold a set number of characters (20 for a last name, 22 for a first name, 45 for a business name). FileIt limits each answer to what fits.
2 Part 2: The designated person
Say who receives the authorization or power of attorney. For an individual, give their name and the last three digits of their SIN. For a business or trust, lines 12 to 12b stay blank: give the business name on line 13 and the name of a contact person there on lines 13a and 13b. Add the address and telephone number, and line 16 if Revenu Québec has given them a professional representative number, which lets them access the file online.
3 Part 3: What it covers
Box 21 grants a general authorization covering all information Revenu Québec holds about the person under Québec tax legislation and the Excise Tax Act. It does not include support payments or the shelter allowance program: tick box 22 (with the support file numbers) or box 23 as well if those should be covered.
For a limited authorization, tick the categories you want in section 3.2.2, from personal income tax returns and the solidarity tax credit to GST/HST and QST returns, source deductions, fuel and tobacco tax, mining tax or other listed documents. For boxes 31 to 37 you can give file numbers; if you do not, all files for that item are covered.
4 Section 3.3: Years or periods
Choose all periods (past, current and future), up to three taxation years with an option for subsequent years, or a date range with an option for subsequent periods.
5 Part 4: Authorization or power of attorney
Tick line 50 for an authorization or line 51 for a power of attorney, and print the name of the person granting it. It takes effect on the date of the signature in Part 5 and stays valid indefinitely, unless you enter an end date on line 52 or it is revoked with form MR-69.R-V.
6 Part 5: Signature
The individual signs, or their legal or authorized representative, such as a tutor, curator, mandatary, liquidator of a succession, trustee or holder of a power of attorney, who must send documents proving their authority and form LM-14-V. For a corporation an officer signs, for a partnership a partner, and for a trust a trustee. Line 71 is for the signer's position, line 72 for the date. The signee cannot be the designated person.
Common mistakes to avoid
- Sending a photocopy: the form says photocopies are not accepted.
- Forgetting to sign Part 5, which the form says gets it returned to you.
- Assuming a general authorization covers support payments or the shelter allowance: those need box 22 or 23 ticked as well.
- Having the designated person sign Part 5, which the form does not allow.
- Filling in both lines 12 and 13 in Part 2: for a business or trust, lines 12 to 12b stay blank.
- Giving a professional representative number without completing line 10 or 10a.
- A Québec corporation filing the English version: the form says corporations established in Québec must file the French version.
After you fill it out
Print the PDF and sign Part 5 by hand. If a legal or authorized representative signs, add the documents proving their authority and form LM-14-V, Information About a Representative. If more than one representative must sign, the form says to attach an appendix with each one's name, signature, position and date.
Mail the signed original to one of the two Revenu Québec addresses printed at the top of page 1. FileIt does not send anything for you.

What is the difference between an authorization and a power of attorney on MR-69-V?
An authorization (line 50) lets the designated person consult your confidential information and documents. A power of attorney (line 51) also lets them act for you with Revenu Québec, for example submitting documents, asking for changes to your file and negotiating.
How long does the authorization last?
The form says it takes effect on the date you sign it and remains valid indefinitely, unless you enter an end date on line 52, you revoke it with form MR-69.R-V, or the information on it changes. It also ends if the person dies or is found incapable of managing their affairs by the Superior Court of Québec.
Can I send a copy or a scan of the form?
The form says photocopies are not accepted. Print it, sign the original by hand and mail it to one of the addresses on page 1.
Does a general authorization cover support payments?
No. Box 21 excludes information under the Act to facilitate the payment of support and the shelter allowance program. Tick box 22 (and give the support file numbers) or box 23 as well if you want those covered.
Is MR-69-V the same as the CRA's authorization form?
No. MR-69-V covers Revenu Québec only. Federal tax matters with the Canada Revenue Agency need the CRA's own authorization.
Can a business use the English MR-69-V?
The form says corporations established in Québec must communicate with Revenu Québec in French and must file the French version (MR-69).
