What is the youth sports code of conduct form template?

Most clubs have a code of conduct; fewer can show who has agreed to it. When there's a problem on the sideline, a signed code makes the expectations, and the consequences, clear to everyone.

This template has four pages. The first identifies the player, date of birth, team and season. The player's code is set out as editable text, then players tick all six promises and sign. The parent's code follows, with sideline promises to tick and a choice about photos and video. The last page explains safeguarding and the club welfare officer, sets out consequences, and asks the parent to agree to both before giving their details and signing.

Each signed code is saved as a PDF in your FileIt vault, so the club can show who has agreed for the season.
