What is the supplier satisfaction survey form template?

Most supplier feedback flows one way: buyers rate suppliers. Asking suppliers how you perform as a customer shows the other side — whether your orders are clear, your forecasts help, your changes are manageable and you pay on time. Suppliers who find you easy to work with may give you better service when capacity is tight.

This survey starts with a short reassurance that answers won't affect orders, an optional company name and a few profile questions. The main page rates nine aspects of working with you, from clarity of orders to treating suppliers as partners, and asks how often invoices are paid within terms, whether forecasts help, how often orders change at short notice, how easy it is to reach the right person and how fair your terms are. The last page covers overall satisfaction, comparison with other customers, open feedback, an ethics question and an optional follow-up.

Each response is saved in your FileIt vault, and the CSV export gives procurement a clear view across all suppliers.

The supplier satisfaction survey form, page by page

1 About you

A short introduction reassures suppliers that answers won't affect orders or contracts. The company name is optional, so suppliers can respond anonymously. They say what they supply, how long they've supplied you and the size of their business.

2 Working with us

A grid rates clarity of orders, ease of ordering, responsiveness, communication of changes, forecast accuracy, lead times, paying invoices on time, resolving disputes fairly and treating suppliers as partners, from very poor to excellent or not applicable. "How often are you paid within the agreed terms?" asks how late payments affect them for anything other than always. Questions on forecasts and short-notice changes follow, with five-point scales for ease of contact and fairness of terms.

3 Overall

Suppliers rate overall satisfaction from 1 to 10, compare you with their other customers and say whether they'd like to keep working with you. Open questions ask what you do well, the one thing to change and ideas to reduce cost, waste or lead times. An ethics question asks about any concerning staff behaviour, with a box for details. Suppliers who'd like a follow-up give their name and email.

Make the template yours

  • Add your ethics or whistleblowing line to the ethics question's help text
  • Edit the grid rows to the parts of the relationship you want to measure
  • Remove the company name to make the survey fully anonymous
  • Add a category or business unit question if suppliers work with several teams
  • Set a closing date for your annual survey window
  • Share a summary of results and actions with suppliers afterwards

Tips for a better supplier satisfaction survey form

  • Send the survey from someone outside the buying team, to encourage honest answers
  • Keep the anonymous option for suppliers who worry about consequences
  • Look closely at payment answers from small suppliers
  • Tell suppliers what you changed as a result — it improves response next time
  • Run it at the same time each year to compare results

Every response becomes a PDF in your vault

Each response is saved in your FileIt vault and the procurement team is emailed. Suppliers don't receive a copy by default.

Use the Responses table and CSV export to compare ratings by supplier type or relationship length, and to follow up with suppliers who asked to talk.

  1. Start from this template. It opens in the FileIt Forms designer — change any question, add pages, set the rules for when questions appear.
  2. Share it. Turn on a public link, or send it to people by email, each with their own link. They don’t need a FileIt account.
  3. Get the answers as PDFs. Each response is saved as a PDF in the vault folder you choose, with uploaded files attached — and listed in a Responses table you can export to CSV.

Supplier satisfaction survey form: frequently asked questions

Is the supplier satisfaction survey template free?

Yes. It's a FileIt Forms starter template, on every plan.

Can suppliers respond anonymously?

Yes. The company name is optional, and name and email are only asked for if they want a follow-up.

How is this different from a supplier evaluation?

A supplier evaluation is your rating of a supplier. This survey asks suppliers to rate you as a customer.

Can I send it to my supplier list?

Yes. Share one public link, or send personal links by email — personal links identify the supplier, so use the public link for anonymity.

Does it connect to our procurement system?

No. Export the responses to a CSV to use them elsewhere.

What should we do with ethics concerns?

Treat them through your normal ethics or whistleblowing process. Add the contact for that process to the question's help text.
