What is the schengen visa document checklist form template?

A Schengen visa application is a stack of separate documents, and each one has its own deadline: the insurance has to cover the exact dates, the flight reservation has to match the itinerary, the bank statements have to be recent, and the invitation letter has to arrive from abroad in time. This Schengen visa document checklist template helps an applicant, or the travel agent or relocation team helping them, see what's ready and what isn't.

The first page captures the trip and the passport: the consulate, appointment date, destination countries, entry and exit dates, the entries requested and the main purpose of the trip. A note explains what consulates usually expect from a passport, while making clear that the consulate's own list is the one that counts.

The second page is the checklist itself, a grid where each document is marked Ready, In progress, Not started or Not needed, followed by sections for travel medical insurance, accommodation and funds. When the purpose is Visiting family or friends, a section for the host's invitation appears first. It's a planning tool, not an application. Every saved copy goes into your FileIt vault as a PDF, so you can compare versions as the application comes together.

The schengen visa document checklist form, page by page

1 Trip and passport

The applicant gives their name, email and nationality, then Consulate or visa centre you're applying to and the appointment date and time. Under The trip, they enter the Main destination country (help text explains how it's usually decided), Other Schengen countries you'll visit, the planned entry and exit dates, Entries you're asking for, and Main purpose of the trip: visiting family or friends, tourism, business, an event, medical treatment, airport transit or something else.

The Passport section asks for the Passport issue date, Passport expiry date and Blank pages left. Have you held a Schengen visa before? opens a field for the most recent visa's country and dates. A short note sums up what consulates usually want from a passport.

2 Documents

If the purpose is visiting family or friends, the page opens with Your host's invitation: the host's name, how you know them, their status in that country, their address and phone, whether the invitation letter is in hand, asked for or not yet asked, and uploads for the letter and a copy of the host's passport or residence permit.

Where is each document? is a grid of ten rows, from the application form and two recent passport photos to the return flight reservation, proof of accommodation, bank statements, a letter from an employer or school, a cover letter and the visa fee. Each row is marked Ready, In progress, Not started or Not needed.

Travel medical insurance records the insurer, policy number, medical cover amount (in euros) and the dates it runs. Accommodation and funds asks where the applicant will stay, the addresses and booking references, how they'll show they can pay for the trip, and the money available. A Still to do box closes the checklist.

Make the template yours

  • Add rows to the document grid for things your consulate asks for, such as a marriage certificate or a parental consent letter
  • Change the currency on the funds fields if your clients think in another currency
  • Add a field for the visa application reference once it's issued
  • Remove the invitation section if your applicants only travel for business
  • Set the vault folder to one per applicant, so each saved version stacks up together
  • Add your agency's contact details to the cover text

Tips for a better schengen visa document checklist form

  • Download the checklist from the consulate you're applying to and compare it row by row
  • Make sure the insurance dates cover the whole stay, including travel days
  • Book refundable accommodation and flight reservations until the visa is granted
  • Ask your host for the invitation letter early, since it may need to be signed or stamped
  • Save a new copy of the checklist whenever a document moves from In progress to Ready

Every response becomes a PDF in your vault

Each submission is saved as a PDF in your vault, titled with the applicant's name and the date, with the invitation letter and host ID attached when they're uploaded. The applicant can get an emailed copy to work from.

Because more than one response is allowed, the applicant can submit an updated checklist as things change, and the Responses table shows every version in order. Agents handling several applicants can export the list to CSV to see who is waiting on what.

  1. Start from this template. It opens in the FileIt Forms designer — change any question, add pages, set the rules for when questions appear.
  2. Share it. Turn on a public link, or send it to people by email, each with their own link. They don’t need a FileIt account.
  3. Get the answers as PDFs. Each response is saved as a PDF in the vault folder you choose, with uploaded files attached — and listed in a Responses table you can export to CSV.

Is this Schengen visa checklist free?

Yes. It's a starter template in FileIt's Forms app, on every plan including free.

When does the invitation section appear?

When the main purpose of the trip is Visiting family or friends. It then asks for the host's details and the invitation letter.

Can I update the checklist later?

Yes. Submit it again from the same link whenever something changes, and each version is saved separately.

Can a travel agent use it for clients?

Yes. Send each client the link or an email request, and every client's checklist arrives as its own PDF.
