What is the religious education enrollment form template?
Every autumn, religious education programs re-enroll dozens or hundreds of children. The coordinator needs to know each child's school grade, which class they belong in, when the family can attend, who may collect them, and how fees will be paid. Parishes also need baptism details for children preparing for First Communion or Confirmation; synagogues and mosques want to know about an upcoming bar or bat mitzvah or a child close to completing the Quran.
This template brings it together in two pages. The first covers the family and the child: grade at school, whether they're new or returning, their class or level (with an option to have them placed), and which scripts they can read. "Is your child preparing for a sacrament or milestone this year?" separates sacrament preparation from other milestones. Choosing sacrament preparation asks whether the child has been baptized, and a yes asks for the church, date and an upload of the baptism certificate.
The second page handles class times, collection, health, learning needs, photo permissions and fees, including a private request for financial help. Each enrollment is filed as a PDF in your FileIt vault, ready for the program office.

The religious education enrollment form, page by page
1 Family and child
The parent gives their name, mobile, email, home address and whether the family is registered with your community, plus an optional second parent. For the child: "Child's name", "Date of birth" and "Grade at school this year". "Has your child studied with us before?" asks new students about any religious education elsewhere. "Class or level" includes "Please place my child", and "Can your child read" lists Hebrew, Arabic and Greek.
"Is your child preparing for a sacrament or milestone this year?" offers sacrament preparation, another milestone such as a bar or bat mitzvah or completing the Quran, or no. Sacrament preparation asks "Has your child been baptized?" and which sacraments they've already received. A yes to baptism asks for the "Church of baptism, with city", the "Date of baptism" and a "Baptism certificate" upload. Another milestone asks which one and the date, if set.
2 Classes, care and fees
"Class times that work for your family" covers weekend mornings and afternoons and weekday sessions. The parent explains how the child will get there, lists other adults who may collect them, notes allergies or medical needs, and shares "Anything that helps your child learn well with us?". "Class photos" offers three levels of permission.
"How would you like to pay the fees?" offers paying in full, instalments, or asking about financial help, which opens a private note. The parent says how many children are enrolling, whether they could help as an aide or substitute, agrees to the "Parent agreement" and signs.
Make the template yours
- Rename the levels to your own class names, such as Kitah Aleph or Juz Amma
- Remove the questions that don't apply to your tradition
- Put your fees, term dates and sibling discount in the cover text
- Edit the class times to your actual schedule
- Add a question for the child's Hebrew or Arabic name
- Set a close date for registration
Tips for a better religious education enrollment form
- Open enrollment before the summer break so you can plan class sizes
- Ask for baptism certificates early — parishes elsewhere can take weeks to send them
- Share allergy notes with each teacher before the first class
- Handle financial help requests privately and promptly
- Use one enrollment per child so each record stays separate
Every response becomes a PDF in your vault
Each enrollment is saved as a signed PDF in your FileIt vault with any baptism certificate attached, and the program office is emailed. The parent receives a copy.
The Responses table lets your coordinator see enrollments by level or class time, and the CSV export helps you build class lists and track who has asked for financial help.
- Start from this template. It opens in the FileIt Forms designer — change any question, add pages, set the rules for when questions appear.
- Share it. Turn on a public link, or send it to people by email, each with their own link. They don’t need a FileIt account.
- Get the answers as PDFs. Each response is saved as a PDF in the vault folder you choose, with uploaded files attached — and listed in a Responses table you can export to CSV.
Religious education enrollment form: frequently asked questions
Is this religious education enrollment form free?
Yes. It's one of the starter templates in FileIt's Forms app, available on every account including the free plan.
Does it work for traditions other than Catholic?
Yes. The milestone question covers a bar or bat mitzvah and completing the Quran, and you can remove or rename any question.
When do the baptism questions appear?
Only when the parent chooses sacrament preparation. A yes to "Has your child been baptized?" then asks for the church, date and certificate.
Can parents enroll more than one child?
Yes. They fill in the form once per child, and the number of children enrolling is asked so you can apply any sibling discount.
Can parents pay fees through the form?
No. The form records how they'd like to pay. Fees are collected through your usual system.
What file types can the baptism certificate be?
PDF, JPG, PNG or HEIC, up to two files.
