What is the product-market fit survey form template?

Early-stage teams often ask whether they've found product-market fit, and one of the best-known ways to check is a single question: how would you feel if you could no longer use the product? Many teams treat a high share of 'very disappointed' answers as a sign they're onto something. The number on its own, though, doesn't tell you who loves the product or why.

This template pairs that question with the follow-ups that make it useful. Users pick very disappointed, somewhat disappointed, not disappointed or I no longer use it. Everyone is asked what they'd use instead, the main benefit they get, the type of person they think would benefit most, and how you could improve the product. Those who've stopped using it are asked why.

Users who answer very disappointed see two extra questions: whether they've recommended it to anyone, and whether they'd introduce you to someone who'd get the same value. A yes asks for their email so you can follow up. Usage frequency, tenure and role round out the page, and every response is filed as a PDF in FileIt.
