What is the personal styling questionnaire form template?
A first styling session goes much further when the stylist already knows the client's sizes, how their week looks, which colours they never wear and what they're hoping to change. Asking all of that face to face uses up time that could be spent shopping or editing the wardrobe.
This personal styling questionnaire gathers it beforehand. The first page covers work, dress code, a typical week and sizes — height, tops, trousers, dresses or suits and shoes — with space for fit notes. The second page explores style: up to three style words, what the client wants to change, pieces they love, colours they love and avoid, prints, anything they'd never wear, favourite brands, openness to second-hand or vintage, and inspiration photos.
The final page asks what you're dressing them for, an event date, typical spend per item, total budget, the service they're interested in and their availability. Each questionnaire is saved as a PDF in your FileIt vault with the photos attached, ready to read before you meet.

The personal styling questionnaire form, page by page
1 About you & sizes
The client gives their name, email and phone, what they do for work and their dress code, from formal to uniform. A checklist describes a typical week — office days, working from home, presenting, outdoors, school runs, travel, evenings out and events.
The Sizes section covers height, tops, trousers or jeans, dresses or suits and shoes, followed by fit notes such as a long torso or often being between sizes.
2 Style & colours
Clients pick up to three style words from classic and minimal to bohemian and creative. What would you like to change about your wardrobe or style? is required, followed by pieces they love wearing and why.
Colours they love and avoid, how they feel about prints, anything they'd never wear, favourite brands and openness to second-hand or vintage follow. Up to 10 inspiration photos can be uploaded.
3 Budget & occasions
What are we dressing you for? is required, covering everyday refreshes, work, interviews, weddings, travel, a new season, a capsule wardrobe or a life or body change, and an event date if there is one.
Typical spend per item and the service they're interested in — online styling, an in-person shopping trip, a wardrobe edit at home or not sure — are required. Total budget, availability and a final box for comfort, mobility or confidence notes complete the questionnaire.
Make the template yours
- Change the per-item budget bands and currency to suit your clients
- Replace the service options with your own packages
- Add questions about body shape or proportions if that's part of your approach
- Add a menswear-specific size set, such as collar and inside leg
- Prefill each client's name and email with a personal link when they book
- Edit the cover message in your own voice
Tips for a better personal styling questionnaire form
- Send the questionnaire when the client books, with a few days to complete it
- Read the never-wear and colours-to-avoid answers carefully — they build trust quickly
- Use the inspiration photos to start the first conversation
- Treat sizes as a starting point; brands vary, so check fit in person
- Ask about the event date early when dressing for an occasion
- Keep each client's questionnaire to look back on before follow-up sessions
Every response becomes a PDF in your vault
Each questionnaire is saved as a PDF in the vault folder you choose, with inspiration photos attached. You're emailed when a client submits, and the client can receive a copy of their answers.
The Responses table lists every client's answers, so you can look up sizes or budgets quickly before a shopping trip, and export to CSV if you keep your own client records.
- Start from this template. It opens in the FileIt Forms designer — change any question, add pages, set the rules for when questions appear.
- Share it. Turn on a public link, or send it to people by email, each with their own link. They don’t need a FileIt account.
- Get the answers as PDFs. Each response is saved as a PDF in the vault folder you choose, with uploaded files attached — and listed in a Responses table you can export to CSV.
