What is the personal loan application form template?
Small lenders, credit unions and employers with loan schemes often still take applications on paper or by email, then spend days chasing a missing payslip or a previous address. This template collects a complete application in one sitting, with the documents attached.
It starts with the loan itself (amount, term and purpose), then the applicant's details and address, with a previous address asked only when they've lived at the current one for less than three years. Employment questions change with the answer: employer and years of service for employees, business name and years trading for the self-employed. Monthly income and outgoings, a credit history question, and uploads for photo ID and proof of income complete the picture.
Two consent statements cover identity and credit checks and make clear that submitting is not an offer of credit. Each application is filed as a PDF in your FileIt vault.

The personal loan application form, page by page
1 The loan
How much would you like to borrow? and Over how long? come first, with terms from 6 to 84 months. What is the loan for? lists common purposes. Other asks for a description, and debt consolidation asks how many debts would be combined. Is this a joint application? explains that the second applicant gets their own form, and a Yes asks for their name and email.
2 About you
The applicant gives their full legal name, date of birth, marital status, email, mobile phone, number of dependants and residential status, then their current address and years at it. Fewer than three years brings up a required previous address.
3 Employment & income
Employment status offers employed, self-employed, retired, student or not working. Employed applicants are asked whether they work full or part time, their employer, job title and years with the employer, and self-employed applicants for their business name and type and years trading. Everyone gives Monthly income after tax, with an optional figure for other monthly income such as pensions or rental income.
4 Outgoings & credit history
Three monthly figures cover rent or mortgage, payments on loans and credit cards, and other regular outgoings. The applicant answers whether they've had a default, judgment, bankruptcy or insolvency arrangement in the last six years, with details if so, and whether they expect their income or outgoings to change in the next year.
5 Documents & consent
Photo ID is required, and proof of income (payslips or bank statements, up to six files) is requested. The applicant agrees to identity and credit checks as explained in your privacy notice, declares the information true, acknowledges that submitting is not an offer of credit, and signs.

Tips for a better personal loan application form
- Tell applicants which documents they'll need before they start
- Respond with a timeline, even if a decision takes a few days
- Never promise approval on the form or the confirmation message
- Ask the second applicant to complete their own form for joint loans
- Limit vault access, as applications hold ID and financial details

Underwriters can search the Responses table by name, open the full PDF, and export to CSV to compare amounts, terms, income and outgoings across applications.
