What is the personal financial statement form template?
Lenders, landlords asking for a guarantor and financial advisors often ask for the same thing: a personal financial statement listing assets, liabilities and income on a given date. People usually piece one together in a spreadsheet, and every version looks different. This personal financial statement template gives everyone the same three pages and the same headings, ending in a signature.
The first page covers the person, their employer and who the statement is prepared for. A joint statement with a spouse or partner adds their name, employer and job title, and a second signature at the end. The second page lists assets: cash and investments, retirement accounts, vehicles, business interests and personal property. Answering yes to Do you own any real estate? opens a section for each property, with its address, type, market value, mortgage balance, monthly payment, lender and any rent received, for up to three properties.
The third page lists liabilities and annual income, and asks a few direct questions about guarantees, lawsuits and bankruptcy. The finished statement is saved as a signed PDF in your FileIt vault, with any statements or valuations attached.

The personal financial statement form, page by page
1 About you
The person gives their full name, date of birth and the statement date, which help text explains is the date the balances and values are taken from. Email, phone and home address are required. Employer, job title and years with this employer follow, then Prepared for, from a loan application or mortgage to a financial advisor, a lease or loan guarantee, or their own records.
Is this a joint statement with a spouse or partner? is required. A yes asks for the spouse or partner's name, their employer and job title.
2 Assets
Cash and investments lists checking accounts; savings, money market and CDs; stocks, bonds and funds at market value; retirement accounts (401(k), IRA, pension); cash value of life insurance; and money owed to you. Other assets covers vehicles, a business you own (your share), personal property and valuables, and other assets.
Do you own any real estate? is required. A yes opens Property 1, with the property address, type (main home, rental property, second or vacation home, land or commercial), market value, mortgage balance, monthly mortgage payment, lender and monthly rent received. Add property 2? and Add property 3? repeat the same section. Total assets is required.
3 Liabilities and income
Liabilities lists credit card balances, auto loans, student loans, personal loans and lines of credit, home equity loans not listed above, unpaid taxes and other debts. Total liabilities is required, with help text reminding the person to include the mortgage balances from the previous page, and net worth follows. Have you co-signed or guaranteed anyone else's debt? asks for details on a yes.
Annual income asks for salary or wages before tax (required), bonus and commission, self-employment income, dividends and interest, rental income, and other income with its source. A few questions asks about lawsuits or judgments and any bankruptcy, each with a follow-up on yes. Statements or valuations can be uploaded. The person agrees to a declaration, signs and dates it. On a joint statement, the spouse or partner signs too.
Make the template yours
- Add a schedule of individual accounts if your lender needs institution names and numbers
- Rename the Prepared for options to match your products
- Add a section for business debts if you lend to business owners
- Edit the declaration to match your own verification wording
- Set the document title to include the applicant's loan reference
- Restrict the vault folder these statements file into, since they hold sensitive figures
Tips for a better personal financial statement form
- Use the same statement date for every figure, ideally a recent month-end
- Value property and vehicles at what they'd realistically sell for, not what was paid
- Include the mortgage balances in total liabilities so net worth isn't overstated
- Attach recent statements for the biggest assets to save follow-up questions
- Ask for a fresh statement if more than a few months pass before a decision
Every response becomes a PDF in your vault
Each statement is saved as a signed PDF in the vault folder you choose, titled with the person's name and date, with any statements or valuations attached. You get an email for every new statement, and the person can get their own copy.
From the Responses table, open any statement to review it, or export to CSV to compare totals across applicants. Because figures are entered field by field, the export gives each asset and liability its own column.
- Start from this template. It opens in the FileIt Forms designer — change any question, add pages, set the rules for when questions appear.
- Share it. Turn on a public link, or send it to people by email, each with their own link. They don’t need a FileIt account.
- Get the answers as PDFs. Each response is saved as a PDF in the vault folder you choose, with uploaded files attached — and listed in a Responses table you can export to CSV.
