What is the partner satisfaction survey form template?
Partners rarely tell their vendor directly that margins aren't competitive, rebates are late or the direct sales team keeps taking their deals. They just put more effort into another vendor's products. A structured, optionally anonymous survey gives them a safe way to say so.
This template asks what type of partner the company is, how long they've been with you, their tier and the respondent's role. It then asks them to agree or disagree with seven statements about programme value, from margins and rebates to deal registration and profitability, and how your programme compares with other vendors'.
An enablement page rates training, marketing materials and funds, the partner portal, channel manager responsiveness, deal and presales support, communication and lead quality, with a question on channel conflict. It ends with satisfaction and recommendation scores, 12-month plans and investment priorities. Each response is saved as a PDF in your FileIt vault.

The partner satisfaction survey form, page by page
1 About your company
The company name is optional, so partners can respond anonymously. They choose their partner type (reseller, distributor, managed service provider, systems integrator, referral partner, technology partner or other), how long they've been a partner, their tier, and their own role, from executive to sales, presales, marketing or operations.
2 Programme value
A grid asks partners to agree or disagree that margins are competitive, incentives are easy to understand, rebates arrive accurately and on time, deal registration protects their deals, pricing and quoting are easy, requirements are reasonable for their tier, and selling your products is profitable.
They then say whether your programme is better, the same or worse than other vendors'. Anyone who says worse can explain what others do better.
3 Enablement & support
A second grid rates sales training, technical training and certification, marketing materials, marketing development funds, the partner portal, channel manager responsiveness, deal support from your sales team, presales support, communication about changes and lead quality. A required question asks whether they've had a conflict with your direct sales team over a deal in the last 12 months; a yes opens a box to describe it.
4 Overall
Partners give overall satisfaction on a 1–10 scale and a 0–10 score for how likely they are to recommend becoming your partner. They say whether, over the next 12 months, they plan to grow their business with you, keep it about the same, reduce it or consider ending the partnership, or aren't sure yet; anyone reducing is asked why. They choose up to three areas to invest in first and can ask their channel manager to follow up, which reveals name and email.
Make the template yours
- Replace partner types and tiers with the names your programme uses
- Add a question about a specific benefit, such as not-for-resale licences or a demo programme
- Split the survey by role, sending a technical version to presales staff
- Add your regions as a dropdown to compare results by territory
- Set the survey to close before your partner advisory meeting
- Send personal links to your partner contacts so you can track who has responded
Tips for a better partner satisfaction survey form
- Tell partners whether responses are anonymous before they start, and keep that promise
- Have someone other than the partner's own channel manager read the conflict answers first
- Follow up quickly with partners who say they plan to reduce their business
- Share a summary of results and planned changes with all partners
- Run the survey at the same time each year so you can compare results
- Don't time it straight after a rebate payment or price change, which can skew the answers
Every response becomes a PDF in your vault
Each response is saved as a PDF in your FileIt vault. With notifications on, the partner team is emailed for each response, so a request for follow-up isn't missed.
From the Responses table you can filter by partner type or plans, and export everything to a CSV with each grid row in its own column for analysis.
- Start from this template. It opens in the FileIt Forms designer — change any question, add pages, set the rules for when questions appear.
- Share it. Turn on a public link, or send it to people by email, each with their own link. They don’t need a FileIt account.
- Get the answers as PDFs. Each response is saved as a PDF in the vault folder you choose, with uploaded files attached — and listed in a Responses table you can export to CSV.
Partner satisfaction survey form: frequently asked questions
Is this template free?
Yes. It's included in the FileIt Forms app on every account, including the free plan, and you can edit, add or remove any question.
Do respondents need a FileIt account?
No. Anyone with the form's link can respond without signing up or signing in.
Can partners respond anonymously?
Yes. The company name is optional, and name and email are only asked for when a partner requests follow-up.
Can we survey several people at each partner?
Yes. The template accepts more than one response, and the role question lets you compare sales, technical and executive views.
Does it connect to our PRM or CRM?
No. Responses are saved as PDFs in FileIt and can be exported to a CSV, but the form doesn't connect to partner or CRM systems.
Can we send personal links instead of a public link?
Yes. You can email personal links to partner contacts. Note that responses linked to a person are no longer anonymous.
How do we handle a reported channel conflict?
The form records what the partner tells you. Your own partner team should follow up and decide how to resolve it.
Can we close the survey on a set date?
Yes. Set a closing date or a maximum number of responses in the form's settings.
