What is the loyalty program sign-up form template?

A loyalty program lives or dies at the moment of sign-up. Ask too much and customers abandon it halfway through; ask too little and you can't send the birthday offer or know which store they visit. This template keeps to one page and asks only what a rewards program actually uses: who they are, how to reach them, when their birthday is, where they shop and what they'd like to hear about.

The birthday question is deliberately light. Customers choose a “Birthday month” and, once they have, a “Day”. There's no year, which is all you need for a birthday treat and less than many people are comfortable sharing.

Marketing preferences are handled with care. “How can we send you rewards and offers?” offers Email, SMS and Post. Ticking SMS reveals a required “Mobile number”, and ticking Post reveals a postal address. A separate, optional marketing consent appears once a channel is chosen, so agreeing to the program terms and agreeing to offers stay two different things. Every sign-up is filed as a PDF in your FileIt vault, giving you a dated record of what each member agreed to.

The loyalty program sign-up form, page by page

1 Join the program

Customers enter their name and email, with an optional “ZIP or postal code”. The birthday month dropdown carries help text explaining it's for a birthday treat and the year isn't needed. “Your usual store” lists example branches plus “I mostly shop online”, and “How did you hear about the program?” covers in store, a receipt, social media, a friend or your website.

“What are you most interested in?” lets members tick new arrivals, member offers, events and tastings, or tips and how-tos. Next come the channel choices and their follow-up fields. The marketing consent reads “Yes, send me rewards and offers by the channels I've ticked. I can opt out at any time.” The required program terms consent closes the form.

Make the template yours

  • Replace the example store names with your own branches
  • Link your program terms in the cover text so members can read them before agreeing
  • Edit the interests to match what you actually send, such as new menu items or classes
  • Remove Post as a channel if you never send printed mailings
  • Change the confirmation message to say how members collect their first reward

Tips for a better loyalty program sign-up form

  • Put the form's link in a QR code at the till — people sign up while they wait
  • Offer something immediate for joining, such as a discount on today's purchase
  • Only contact members by the channels they ticked
  • Export new members weekly and import them into your email or SMS tool
  • Remove anyone who asks to opt out promptly, and keep a note of when they asked

Every response becomes a PDF in your vault

Every sign-up is saved in your vault as a PDF showing exactly which channels were ticked and which consents were given, and you're emailed each time someone joins. The new member receives a copy by email, a handy confirmation that they're in.

The Responses table becomes your member list. Filter by store or search by name, and export to CSV to load members into your mailing tool or point-of-sale system.

  1. Start from this template. It opens in the FileIt Forms designer — change any question, add pages, set the rules for when questions appear.
  2. Share it. Turn on a public link, or send it to people by email, each with their own link. They don’t need a FileIt account.
  3. Get the answers as PDFs. Each response is saved as a PDF in the vault folder you choose, with uploaded files attached — and listed in a Responses table you can export to CSV.

Loyalty program sign-up form: frequently asked questions

Is this loyalty sign-up template free?

Yes. It's a ready-made starter in FileIt's Forms app, available on every account, including the free plan.

Does FileIt track points or rewards?

No. The form collects sign-ups and consent. Points and rewards are managed in your till, app or loyalty system.

Why is marketing consent separate from the terms?

So joining the program and agreeing to receive offers are two clear choices. Members can join without opting in to marketing.

Why only birthday month and day?

That's all you need to send a birthday offer, and many customers prefer not to share their age.

Can I run it on a tablet in store?

Yes. Open the public link on a tablet and leave it on the counter. The form allows more than one response from the same browser.

Do members need an account?

No. They fill in the form and submit — no sign-up with FileIt.
