What is the influencer collaboration application form template?

Once a brand gets noticed, creator DMs start arriving: a handle, a follower count, sometimes a rate card, rarely the same information twice. Comparing them side by side is close to impossible. This influencer collaboration application asks every creator the same questions, so you can see channels, audience and ideas in one consistent format before you reply.

The first page is about the creator and their channels: the creator or channel name, “Platforms you post on”, the main handle or channel link, followers on the main platform, average engagement rate and views, how often they post, up to three niches and a description of their audience. They can upload screenshots of their analytics so you aren't relying on self-reported numbers alone.

The second page is the collaboration. “What kind of collaboration are you after?” offers Paid, Gifted (product in exchange for content), Affiliate commission or Open to any. Paid shows a short rate card, with prices for a feed post, a three-frame story set, short-form video and long-form or dedicated video, plus the usage rights those rates include and an optional media kit upload. Gifted skips the rates and asks which products they'd like to try. Every application is filed as a PDF in your FileIt vault.

The influencer collaboration application form, page by page

1 You & your channels

The creator gives their name, “Creator or channel name”, email, phone and “Where you're based”. They tick the platforms they use, from Instagram and TikTok to Twitch, LinkedIn and podcasts, and enter their main handle plus any other links. Number fields capture followers, engagement rate and average views, and a dropdown covers posting frequency.

“Your niche” takes up to three choices, with an Other option. “Describe your audience” asks about age range, top locations and why people follow them, and “Audience insight screenshots” accepts up to four images or a PDF.

2 The collaboration

After choosing the collaboration type, paid creators see “Your rates” and the usage rights question. Gifted creators are asked which products they'd like to try.

Everyone answers “Your idea for working with us” and ticks “Content you could create”, from feed posts and stories to live streams and content for your own channels. “Have you worked with brands before?” asks for brands and links when the answer is yes, and an optional field notes any current exclusivity deals. Two consents close the form: one to label paid, gifted or affiliate content clearly, and one confirming that the application isn't an agreement.

Make the template yours

  • Swap the platforms and niches for the ones that matter to your brand
  • Change the rate fields' currency, or add a rate for UGC-only content
  • Set a minimum follower count in the help text if you have one
  • Remove the affiliate option if you don't run an affiliate program
  • Close the form automatically once you have enough applications for a campaign

Tips for a better influencer collaboration application form

  • Check the analytics screenshots against what you can see on the creator's profile
  • Weigh engagement and audience fit above raw follower counts
  • Reply to everyone, even with a short no — creators talk to each other
  • Agree deliverables, deadlines and usage rights in writing before any content goes live
  • Keep a note of which creators you gifted, so you can follow up after they post

Every response becomes a PDF in your vault

When a creator submits, FileIt files the application and uploaded screenshots as a PDF document in your vault and emails you. The creator gets a copy of what they sent.

In the Responses table, search by platform, niche or handle to shortlist creators for a campaign, and export to CSV to compare rates side by side.

  1. Start from this template. It opens in the FileIt Forms designer — change any question, add pages, set the rules for when questions appear.
  2. Share it. Turn on a public link, or send it to people by email, each with their own link. They don’t need a FileIt account.
  3. Get the answers as PDFs. Each response is saved as a PDF in the vault folder you choose, with uploaded files attached — and listed in a Responses table you can export to CSV.

Influencer collaboration application form: frequently asked questions

Is this influencer application template free?

Yes. It's one of the starter templates in FileIt's Forms app, which every account has, including the free plan.

Do creators need a FileIt account?

No. Share the public link in your bio, on your website or in a reply to a DM, and creators apply without signing up.

Why don't gifted applicants see the rate card?

Rates only make sense for paid work, so the rate fields appear only when the creator chooses Paid.

Can creators upload their media kit?

Yes. Paid applicants can upload a rate card or media kit, and anyone can upload analytics screenshots.

Does submitting create a contract?

No. The form says clearly that any collaboration will be confirmed in writing with its own brief and terms.

How is this different from the affiliate application?

This one is about content collaborations: rates, deliverables and ideas. The affiliate template focuses on commission-based partnerships.
