1 About you
The creator gives their name, email, phone, creator or brand name and country, and says whether they're applying as an individual, a business or an agency — agencies name the creator they represent. They confirm whether they're 18 or older; answering no shows a note that under-18s need a parent or guardian to get in touch first.
2 Your channels
A checklist covers Instagram, TikTok, YouTube, blogs, newsletters, podcasts and more, followed by a link to the main channel, other links and a combined audience size. Ticking blog or newsletter opens questions for monthly visitors or subscriber numbers.
3 Audience & content
Creators pick up to three niches, describe where their audience lives and its main age group, share typical engagement and can upload an analytics screenshot. They say whether they've used your products, explain how they'd promote you and list other programs they work with.
4 Payout & agreement
The creator chooses a payout method and whether they're open to gifted product. Two consents close the form: one to disclose the affiliate relationship or gifted product in every post, and one to the program terms, including no paid search bidding on your brand name, no misleading claims and no unapproved coupon posting.

- Reply to rejected applicants too; many will grow and reapply

Yes. Choosing 'An agency on behalf of a creator' asks for the name of the creator they represent.
