What is the employee commute survey form template?
Car parks fill up, bus routes change, and teams ask for bike racks or a shuttle. Decisions like these are easier when you know how many people drive, how far they come and what they'd switch to. A commute survey gives you those numbers in one go.
This template is a single page. Staff give their site and days on site, their main way of getting to work, distance and time, what it costs each week and how stressful the trip feels. They tick which alternatives they'd consider, from a discounted transit pass to flexible start times. People who drive alone get three extra questions on carpooling and parking.
No names are needed. Each response is filed as a PDF in your FileIt vault, and the answers collect in the Responses table so you can export them for a travel plan, a parking review or a sustainability report.

The employee commute survey form, page by page
1 Your journey to work
Staff start with the site or office they mostly work from and "Days a week you travel to work". The required question "How do you usually get there? Choose the longest part of the trip" offers drive alone, carpool, bus or train, cycle, walk, motorbike, or Other. Choosing "Drive alone" opens three more: "Would you share a ride with a colleague who lives nearby?", their neighbourhood or nearest town for matching (no street address), and whether they need a parking space every day on site. Everyone then ticks any other ways they travel in a typical week and picks a one-way distance band shown in both miles and kilometres. They give their usual one-way time in minutes and, optionally, roughly what commuting costs them a week in fuel, fares, parking and tolls. A 1–5 scale asks "How does your commute feel most days?", from easy to very stressful. The page closes with "Which of these would you consider using?", covering transit passes, a cycle scheme, bike storage and showers, a shuttle, EV charging, flexible hours and more work-from-home days, and an open box for what would make getting to work easier.
Make the template yours
- Turn the site question into a dropdown of your locations
- Change the currency on the weekly cost question to yours
- Remove alternatives you can't offer, so you don't raise expectations
- Add a question on shift start times if you run early or late shifts
- Rename transport options to local terms, such as subway or metro
- Set a close date to capture a single snapshot
Tips for a better employee commute survey form
- Run it in a normal week, not during school holidays or bad weather
- Compare carpool interest with home areas before promising a matching scheme
- Count drivers who need a daily parking space alongside days on site to size the car park
- Look at the stress scale by travel mode to see who's struggling most
- Share what you'll change as a result, even if it's small
- Repeat the survey after a change to see whether habits shifted
Every response becomes a PDF in your vault
Each response is saved as a PDF titled with the date in the vault folder you pick, and you get an email for each one. No respondent copy is sent.
Export the Responses table to CSV to count travel modes, average travel time or add up weekly costs, and filter on drivers to see who's open to carpooling.
