What is the debt validation request form template?
A letter from a collection agency often names a company you've never heard of, an amount that doesn't match anything you remember, and a deadline. Before paying, disputing or ignoring it, many people want the collector to show where the debt came from and how the figure was worked out.
This template turns that request into a clear, signed page. It records the collector's name and address, the account reference and amount from their letter, and the original creditor if one was named. The person chooses their position: disputing the debt (which asks why), not recognising it, or simply asking for verification. Then they tick what they want sent: an itemised breakdown, the original creditor's details, the signed agreement, proof of ownership, proof of licensing or the last statement.
The finished request is saved as a dated PDF in your FileIt vault, alongside the collector's letter if it was uploaded. That gives you, or the client you're helping, a copy to send and a record of what was asked for and when.

The debt validation request form, page by page
1 Your details
Name, the mailing address where the collector should reply, email and an optional phone number.
2 The collector
The collection agency or law firm's name and mailing address, the date of their first contact and how it came — a letter, phone call, email or court papers. Choosing court papers shows a note that there may be a separate court deadline and that legal help is worth getting quickly.
3 The debt
"Account or reference number they used", "Amount they say you owe" and the original creditor. Then "Your position on this debt": choosing "I dispute this debt" asks why, with examples like already paid or identity theft. "What are you asking them to send you?" lists six common items to tick. The person can also ask for contact in writing only and upload the collector's letter.
4 Sending your request
How the request will be sent — certified mail with return receipt, regular mail, email or portal — the date, a declaration that it isn't an acknowledgement of the debt, a signature and the date signed.
Make the template yours
- Add your organisation's name and a contact line to the cover text if you're helping clients
- Remove the licensing item if it doesn't apply where your clients live
- Add a question about which credit reports show the debt
- Send each client's requests to their own folder in your vault
- Switch off the emailed copy if clients use shared email accounts
- Change the currency if you work outside the US
Tips for a better debt validation request form
- Send the request soon after the collector's first contact — some laws give consumers a time window to ask
- Keep proof of sending, such as a certified mail receipt, with the PDF
- Note every call and letter that follows
- If court papers have arrived, deal with the court deadline first and get legal help
- Rules on validation and disputes differ by country and state, so check what applies or ask an advisor
Every response becomes a PDF in your vault
The signed request is filed as a PDF in the vault folder you choose, with the collector's letter attached, and the owner gets an email. The person who filled it in is emailed a copy they can print and send.
The Responses table shows each request with its send date and collector, so an advisor can see which replies are still outstanding. FileIt doesn't send anything to the collector — the request goes out by the method chosen.
- Start from this template. It opens in the FileIt Forms designer — change any question, add pages, set the rules for when questions appear.
- Share it. Turn on a public link, or send it to people by email, each with their own link. They don’t need a FileIt account.
- Get the answers as PDFs. Each response is saved as a PDF in the vault folder you choose, with uploaded files attached — and listed in a Responses table you can export to CSV.
