What is the cruise booking inquiry form template?

Cruise quotes depend on a lot of details: dates and how flexible they are, the region, the line, the cabin type, how many people are sharing and how old they are. Ask for them one email at a time and the client loses interest. This cruise booking inquiry asks for all of it in one go, in a way that feels like a conversation rather than an application.

The first page covers the cruise itself: earliest departure and latest return dates, how flexible those dates are, cruise length, departure port, where they'd like to sail, cruise lines they like or want to avoid, what matters most on board and the cabin type. The second page is about who's travelling, with names and ages, any celebration, loyalty numbers and a budget. Ticking yes to "Need flights to the ship?" adds the departure airport, flight class and a pre-cruise hotel night.

Each inquiry arrives as a PDF in your FileIt vault with an email alert, so the full brief is in one place when you start searching for sailings.

The cruise booking inquiry form, page by page

1 Your cruise

The client starts with their name, email, phone and the best way to reach them. Then come the earliest departure date, latest return date and "How flexible are your dates?", from fixed to very flexible. They choose a cruise length, name a preferred departure port and tick where they'd like to sail, from the Caribbean and Mediterranean to Alaska, river cruises in Europe or expedition trips. They can list cruise lines they like or want to avoid, tick what matters most on board (family activities, adults-only areas, fine dining, shore excursions, a small ship and more), say whether they've cruised before, choose a cabin type or ask you to advise, and say how many cabins they need.

2 Passengers and extras

The second page asks for the number of adults and children, then the names and ages of everyone travelling. Age matters for fares and kids' clubs. There are fields for an occasion, cruise loyalty numbers, a total budget and whether that budget covers the cruise only or cruise, flights and hotels.

"Need flights to the ship?" opens the departure airport, flight class and a question about a hotel night before sailing. A yes to mobility, medical or dietary needs opens a box to explain what would help. The client can ask for a travel insurance quote, add any notes and confirm they're happy to be contacted, understanding that nothing is booked until they confirm and pay.

Make the template yours

  • Change the budget currency to your own
  • Trim or add regions to match the cruise lines you sell
  • Remove the flights section if you only sell cruise-only fares
  • Put your agency's response time in the confirmation message
  • Send notifications to a shared inbox so any agent can pick up a new inquiry
  • File inquiries into a vault folder per season or per client

Tips for a better cruise booking inquiry form

  • Reply quickly, even with "I'm on it". Cruise clients often ask several agents at once
  • Use passenger ages to check cabin occupancy rules before quoting
  • Offer two or three options at different prices rather than one
  • Ask about the occasion. A celebration is a reason to suggest an upgrade or a special dinner
  • Check passport validity for everyone when you confirm, especially on itineraries with several countries

Every response becomes a PDF in your vault

The inquiry is saved as a PDF in your FileIt vault and you get an email straight away. The client gets a copy of what they asked for, so you're both working from the same brief.

In the Responses table you can see every cruise inquiry with dates and regions, sort by departure date and export to CSV if you track leads in a spreadsheet.

  1. Start from this template. It opens in the FileIt Forms designer — change any question, add pages, set the rules for when questions appear.
  2. Share it. Turn on a public link, or send it to people by email, each with their own link. They don’t need a FileIt account.
  3. Get the answers as PDFs. Each response is saved as a PDF in the vault folder you choose, with uploaded files attached — and listed in a Responses table you can export to CSV.
