What is the credit limit increase request form template?
When a good customer's orders outgrow their credit limit, the request often arrives as a one-line email that leaves the credit controller chasing for accounts, references and an explanation. Meanwhile orders sit on hold.
This form collects the whole request at once: the account and contact, the current and requested limit, whether the increase is permanent or temporary, the reason and expected monthly spend, then business type, turnover, profit, recent changes and any arrears, with an upload for accounts. It finishes with bank and trade references and a signed authorisation to contact them and run credit checks.
Each request is filed as a PDF in your FileIt vault with the accounts attached, so the credit team has a complete record of what was asked, what was supplied and when.

The credit limit increase request form, page by page
1 Your account
The customer gives the company name, account number and the name, position, email and phone of the person asking.
2 The request
Enter the current limit and the limit requested, and say whether the increase is permanent or temporary; temporary increases ask for an end date. The customer ticks the reasons, explains more, gives their expected monthly purchases and says which payment terms they'd like.
3 Financial information
Record the business type, years trading, annual turnover, net profit or loss, financial year end and number of employees. Changes in ownership or structure, or any overdue payments, court judgments or insolvency proceedings, open boxes for an explanation. Recent accounts, management accounts or bank statements are uploaded here.
4 References and authorisation
Add the bank and two trade references, say whether a guarantee could be offered, and sign an authorisation for you to contact the references and bank and run credit checks.

Tips for a better credit limit increase request form
- Compare the requested limit with the customer's actual payment record with you, not just their accounts
- Ask for management accounts if the latest annual accounts are more than a few months old
- Set a review date for temporary increases so they don't quietly become permanent
- Tell the customer clearly what limit and terms you've approved, in writing
- Keep the request with the decision for your next credit review
Every response becomes a PDF in your vault
The request is saved as a signed PDF in your FileIt vault with the uploaded accounts, and your credit team can be emailed when it arrives. The customer can get a copy of what they submitted.
Use the Responses table to see every pending request and export to CSV for your credit committee or monthly review.
- Start from this template. It opens in the FileIt Forms designer — change any question, add pages, set the rules for when questions appear.
- Share it. Turn on a public link, or send it to people by email, each with their own link. They don’t need a FileIt account.
- Get the answers as PDFs. Each response is saved as a PDF in the vault folder you choose, with uploaded files attached — and listed in a Responses table you can export to CSV.
