What is the commercial insurance risk profile form template?

Before an insurer can quote for a business, it needs a picture of the risk: what the business does, where it operates from, how the premises are protected, how many people work there, what it turns over and what has gone wrong before. Brokers often gather this over several calls and emails, and still find a gap when an insurer asks a question.

This template collects it in one sitting across five pages: the business and its activities, premises and security, people and finances, claims and insurance history, and the covers wanted with values, start date and current insurer. Questions that don't apply stay hidden, such as premises details for a business with no fixed premises.

Each completed profile is saved as a signed PDF in your FileIt vault with the client's current policy schedule attached, ready to send to insurers and to keep on file for renewal.

The commercial insurance risk profile form, page by page

1 The business

Record the business and trading names, contact, position, email and phone, legal structure, years trading and a full description of activities. A checklist flags higher-risk activities such as work at height, hot work, working away from premises, handling customers' goods, professional advice, food and drink, exporting, subcontractors and hazardous substances.

2 Premises

Enter the main premises address and type. Unless the business is mobile with no fixed premises, the form asks whether the premises are owned or rented, the approximate year built, construction type and the security and fire protection in place. Other premises or sites can be listed.

3 People and finances

Record employees and how many are part-time or seasonal, annual wage roll, turnover for the last year and the next 12 months, and the share of turnover from abroad. If the business uses subcontractors, it's asked for the annual subcontractor spend.

4 Claims and history

The client says whether there have been any claims or losses in the last five years, whether an insurer has ever declined, cancelled or imposed special terms, and whether there are unspent convictions, bankruptcies or disqualifications. Each yes opens a box for details.

5 Cover wanted

Tick the covers to be quoted, from liability and property to cyber, directors and officers and tools. Contents and buildings covers ask for values. Add the liability limit wanted, start date, current insurer and premium, upload the current policy schedule, add anything else the insurer should know, and sign the declaration.

Make the template yours

  • Edit the cover list to the products you place most often
  • Add trade-specific questions, for example for construction, hospitality or professional services
  • Change the currency on the money fields
  • Adjust the claims period from five years if your insurers ask for a different period
  • Replace the declaration with your own firm's wording and privacy notice
  • File each client's profile into their own vault folder

Tips for a better commercial insurance risk profile form

  • Ask clients to describe every activity, including occasional ones; unexpected activities are a common cause of claim disputes
  • Check turnover and wage roll figures against the latest accounts
  • Go through the history questions with the client rather than leaving them to guess
  • Keep the signed profile with the quotes so you can show what was disclosed
  • Send the form again at renewal and ask clients to update anything that's changed

Every response becomes a PDF in your vault

Each risk profile is saved as a signed PDF in your FileIt vault with any uploaded schedules, and you can be notified by email when a client completes it. The client can receive a copy.

Use the Responses table to track profiles by start date or cover type and export to CSV when you prepare submissions to several insurers.

  1. Start from this template. It opens in the FileIt Forms designer — change any question, add pages, set the rules for when questions appear.
  2. Share it. Turn on a public link, or send it to people by email, each with their own link. They don’t need a FileIt account.
  3. Get the answers as PDFs. Each response is saved as a PDF in the vault folder you choose, with uploaded files attached — and listed in a Responses table you can export to CSV.
