What is the collections call log form template?

Collections work depends on follow-through. A customer promises to pay on Friday, Friday comes, and the question is whether anyone remembers the promise, the amount and who made it. Notes scattered across email and spreadsheets make that harder than it should be.

This three-page log is quick to fill in after each call. The credit controller records the date, call type, customer, account number and who they spoke to, then lists the invoices discussed, the total overdue and the oldest item. The reason for delay covers the usual answers — invoice not received, waiting for approval, a dispute, a missing PO, cash flow, the next payment run, or a claim it's already paid — with follow-up questions where they matter.

The outcome page captures a promise to pay with amount, date and method, or a proposed plan, documents sent after the call, the next action and its date, and factual notes. Each call is saved as a PDF in your FileIt vault, building a history for every account.

The collections call log form, page by page

1 Call

The log starts with Logged by, Date of call, Time and Call type: outbound, inbound, voicemail left or no answer. Customer and Account number are required. Spoke to and Their role appear only when someone was reached, not for a voicemail or no answer, and Number called is optional.

2 Invoices & reason

Invoices discussed takes one line per invoice with number, date, amount and days overdue. Total overdue is required, and Oldest item, days overdue is optional. When someone was reached, Reason given for delay lists the common answers; choosing Disputes the invoice asks what is disputed, and Says it's already paid asks for the payment date and reference they gave.

3 Outcome & next step

Outcome offers promise to pay, payment plan proposed, paid during the call, dispute raised, call back requested, refused to pay, or no contact made. A promise to pay asks for amount, date and payment method; a plan asks for its terms. Sent after the call lists invoice copies, statement, proof of delivery, reminder and final notice. Next action and its date, Call notes and Customer's attitude complete the log.

Tips for a better collections call log form

  • Always ask for a specific date and amount — "next week" isn't a promise to pay
  • Check the next action date daily and call the same day a promise is missed
  • Resolve disputes quickly; they're often the real reason for late payment
  • Keep notes factual and courteous
  • Follow local rules on debt collection contact, especially with consumers
