What is UI-2.5 Dependant's benefits (spouse)?

When someone who contributed to the Unemployment Insurance Fund dies, their dependants can claim dependant's benefits under section 30 of the Unemployment Insurance Act. The UI-2.5 is the claim form for the surviving spouse or life partner. Other dependants, such as children, use the UI-2.6 instead.

The single landscape page asks for the deceased contributor's ID number, date of birth, names, date of death and last address, then your own details as the surviving spouse or partner, and ends with your declaration that you were not divorced from the deceased. Your bank details go on the separate UI-2.8 form, as the UI-2.5 itself says.

FileIt asks the questions one at a time, puts each digit of both ID numbers in its own box, prints the dates in the form's day, month and year boxes and checks that the ID numbers are valid before you print.

The 2019 edition has 1 page. FileIt asks for it in 3 parts:

  1. A. Particulars of the deceased contributorSurviving spouse or life partner
  2. B. Particulars of the surviving spouse or life partnerSurviving spouse or life partner
  3. DeclarationSurviving spouse or life partner

How to fill out UI-2.5 Dependant's benefits (spouse), step by step

1 Part A: The deceased contributor

Enter the deceased's ID number (one digit per box), date of birth, gender, first names and surname, date of death and last residential address with its postal code. If they ever claimed from the UIF under a different name or ID number, give those details too.

2 Part B: You

Enter your own ID number, date of birth and gender, then your names, postal address, residential address, phone numbers, occupation and e-mail address. The postal address box is small, so keep it short.

3 More than one surviving spouse

The declaration reads 'I am one of ___ surviving spouses or the only surviving spouse or life partner'. Give the number only when the deceased left more than one spouse; otherwise leave it blank.

4 Date and signature

FileIt prints the date in the day, month and year spaces. You sign by hand on the line marked 'Signature of applicant'. The shaded strip at the bottom is for the labour centre.

5 Banking details

Complete the UI-2.8 so the UIF knows where to pay the benefit. FileIt has the UI-2.8 too.

Common mistakes to avoid

  • Using the UI-2.5 for a child or other dependant, who claims on the UI-2.6.
  • Forgetting the UI-2.8 banking details form.
  • Coming without the marriage certificate or proof of the customary marriage or religious union.
  • A typing mistake in either ID number. FileIt checks the check digit of a 13-digit ID.
  • Filling in the shaded official section, which is for the claims officer.

After you fill it out

Print the page, sign it and take it, with the UI-2.8 and your supporting documents, to a labour centre of the Department of Employment and Labour.

A claims officer checks the claim and completes the shaded section, approving or refusing it.

FileIt keeps a copy of the completed form in your vault, next to the death certificate, so you have it if the UIF asks questions later.

UI-2.5 Dependant's benefits (spouse): frequently asked questions

Who can claim dependant's benefits from the UIF?

The Unemployment Insurance Act lets the dependants of a deceased contributor claim. The surviving spouse or life partner uses the UI-2.5. Other dependants use the UI-2.6.

What if there is more than one surviving spouse?

Each spouse completes their own UI-2.5, and the declaration asks how many surviving spouses there are.

Where do I put my bank details?

On the separate UI-2.8 form. The UI-2.5 says so just above the declaration.

Do I need a marriage certificate?

The form says a surviving spouse needs a marriage certificate or proof of the customary marriage or religious union.

Can I sign electronically?

FileIt prints the form for you to sign by hand, like the other UIF claim forms, because you hand it in at a labour centre.

Does FileIt submit the claim to the UIF?

No. FileIt fills in the Department's official PDF. You hand it in yourself.
